脐带静脉移植在手部肌腱损伤修复中的临床应用

摘　　要：日的：探讨胎儿脐带静脉（NUV）移植行腱鞘重建对手部肌腱粘连的预防作用。方法：1996年～1999年间共收治16例24指手部用肌腱及腱鞘损伤的病例，新鲜损伤9例13指，陈旧损伤7例11指，在进行肌腱修复时，应用NUV移植代替腱鞘包绕或套接于损伤的肌腱段。结果；按TAM法评定疗效，新鲜损伤9列13指，优7指，良5指．可1指；陈旧性损伤7例11指，优5指，良4指，可2指。优良率87．5%。结论：NUV取材方便，植入体内后无抗原性，可同化为相应的正常组织，应用于手部肌腱损伤的修复术中替代腱鞘组织，能较好地预防术后肌腱的粘连。Abstract:Objective:To investigate the preventive effect of newborn umbilical vein(NUV)graft for tendon sheath reconstruction ontendon adhesion. Methods:Finger's tendon and tendon sheath injuries in 24 fingers of 16cases,which were trected from 1996 to 1999,were analyzed. Fresh injuries in 13 fingers of 9 cases,old injuries in 11 fingers of 7 cases, NUV graft was used during tendon repair toreplace the encysted tendon sheath or invagination on injured tendon. Results:According to TAM standard,of the fresh 13 fingers, theresult was excellent in 7. good in 5, and fair in 1;of the old injured 11 fingers, the outcome was excellent in 5.good in 4 .and fair in 2.The overall excellent or good rate was 87. 5 %. conclusion:There are countiful resources of NUV, it is eaay to get the right NUV, andthere will be no antigenicity after the transplantation of NUV, because NUV can be assimilated to be relevent normal tissue. As thesubstitution of tendon sheath tissue in the hand tendon reconstruction, NUV can prevent the adhension of the tendon.
