摘  要:目的 :探讨血浆神经肽 Y水平与冠心病患者冠状动脉病变程度及左心室功能的关系。方法 :应用放射免疫法检测 4 8例选择性冠状动脉造影主要分支狭窄程度大于 50 %的患者和 1 0例冠状动脉造影正常者的血浆神经肽 Y浓度 ,并作冠状动脉病变程度记分和测定左心室射血分数 ,研究血浆神经肽 Y浓度与冠状动脉记分及左心室射血分数之间的关系。结果 :冠心病患者血浆神经肽 Y水平 ( 3 48.4± 4 6.8) ng/ L明显高于对照组 ( 2 2 4 .2± 4 2 .5) ng/ L( P<0 .0 1 ) ;血浆神经肽 Y水平与冠状动脉狭窄程度记分呈显著正相关 ( r=0 .64 ,P<0 .0 1 ) ,而与左心室射血分数呈显著负相关 ( r=0 .59,P<0 .0 1 )。结论 :冠心病患者血浆神经肽 Y水平与冠状动脉病变严重程度和左心室功能均有一定关系 ,血浆神经肽 Y水平升高的冠心病患者左心室功能随冠状动脉病变的加重而减退。神经肽 Y可能参与了冠状动脉粥样硬化性心脏病的发生发展过程。Objective:To investigate the relationship between plasma neuropeptide Y (NPY) levels and the severity of coronary artery atherosclerosis as well as left ventricular function in patients with coronary atherosclerotic heart disease.Methods:Radioimmunoassay was applied to determine NPY levels in plasma.This study included 48 patients with fixed stenosis coronary artery≥50% of luminal diameter in the dominant coronary arteries,and 10 healthy volunteers served as controls.The relationship between plasma NPY levels and the coronary artery score and left ventricular ejection fraction were assessed.Results:Plasma NPY levels in coronary atherosclerotic group and control group were (348 4±46 8) ng/L and (224 2±42 5) ng/L,respectively,and the difference was significant ( P <0 01).Plasma NPY levels were positively correlated with the coronary artery score ( r =0 64, P <0 01),and negatively correlated with left ventricular ejection fraction ( r =0 59, P <0 01).Conclusions:NPY levels in plasma might be related to both the severity of coronary artery atherosclerosis and left ventricular function in patients with coronary atherosclerotic heart disease.The parallel trend between the increase in plasma NPY level and the severity of coronary artery atherosclerosis suggests that NPY appears to be involved in the progression of coronary atherosclerotic heart disease and deterioration of left ventricular function.
