摘  要:目的:评价三维梯度回波序列MR数字减影血管造影对主动脉及分支病变的诊断价值。方法:我们在中场强磁共振机上应用3D快速梯度回波序列3D-FFE对46例患者进行三维动态磁共振血管成像数字减影3DDCEMRDSA造影剂为02mmol/kg。根据试验注射靶血管内时间-信号强度曲线,确定靶血管内信号曲线峰值前端时间为3DDCEMRDSA扫描开始时间。注药前进行第一个动态扫描作为减影蒙片,注药后进行二次动态扫描。原始图像先进行减影处理,再进行最大信号投影MIP重建。结果:46例包括21例正常均满意显示了感兴趣区的血管。经减影后第一次扫描图像动脉显示良好,静脉影浅淡。第二次扫描图像主动脉显影较前变淡。结论:3DDCEMRDSA是显示主动脉及分支病变的理想方法。部分图像可与常规DSA相媲美。Objective:To assess the value of technique of 3D dynamic contrast-enhanced subtraction MRA (3D DCE MRDSA) in the diagnosis of the lesions of the aorta and its major branches.Methods:The technique of 3D DCE MRDSA was designed based on the 3D-FFE sequence in the middle magnetic field system and applied among 46 patients.The mask imagines obtained from the sequence scanning before injection were subtracted by the enhanced images from the two sequence scanning after injection,both groups of subtracted images were processed by MIP technique.Results:Vessels in the region of interest were demonstrated satisfactorily among 46 cases.Arteries were demonstrated clearly on the first group of subtracted imagines,and venes were clearly shown on the second group of subtracted imagines.Conclusion:3D DCE MRDSA is a safe and noninvasive method in diagnosing the lesions of aorta and its major branches.The quality of 3D DCE MRDSA was comparable to that of X-ray DSA.
