小儿先天性心脏病术后机械辅助通气管理体会

摘　　要：笔者报道 1997年 1月～ 2 0 0 0年 4月 ,对 36例小儿先天性心脏病术后行机械辅助通气的管理体会。根据小儿呼吸道解剖及生理特点设置通气参数 ,如呼吸频率、潮气量 ,并因人而异加用呼气末正压通气 (PEEP)等。在辅助通气过程中密切观察患儿生命体征变化 ,定时了解血气变化 ,做好气道护理和患儿心理护理 ,努力做到人机协调 ,掌握撤机指征 ,以保证患儿安全脱机。本组患儿在辅助通气过程中无并发症发生。Mechanical ventilation was used in 36 children patients receiving congenital heart disease operation (average age: 7.8 years old) from January 1997 to April 2000. The parameter of ventilation such as RR and VT was adjusted complying with the anatomical and physiological characters of the children's airway. Positive Expiratory End Pressure (PEEP) was also applied according to the appropriate conditions. Close observation of their life symbol and their blood-gas was performed. The care of airway and the children themselves mentally was done simultaneously to ensure the harmony between the machine and children. The removal of the machine should be based on the right conditions to ensure children's safety. In this study, there were no complications occurring in the process of mechanical ventilation.
