摘 要:目的 通过检测丙型肝炎病毒 (HCV)感染孕妇的羊水中HCVRNA ,探讨HCV母婴传播的途径及HCV基因型。方法 应用荧光定量聚合酶链反应 (PCR)和逆转录巢式PCR(RT nPCR)技术 ,检测孕妇血清和羊水中的HCVRNA。对 34例 (研究组 )血清HCVRNA阳性标本采用酶切分型法检测 ,依据限制性片段长度多态 (RFLP)图谱判断HCV基因型。另选 40例正常孕妇 (对照组 )的血清和羊水作为对照。结果 在研究组孕妇羊水中仅有 2例 (5 9% )检测出HCVRNA ,病毒滴度分别为 10 5、10 6拷贝 /ml;对照组孕妇血清和羊水标本中均未检测出HCVRNA。研究组孕妇血清中HCVRNA阳性标本HCV基因分型为 :Ⅱ型 2 7例 (79 4% ) ,Ⅲ型 5例 (14 7% ) ,Ⅱ /Ⅲ混合型 2例 (5 9% )。结论 HCV感染孕妇的羊水中HCVRNA阳性率极低 ,提示羊水不是HCV母婴传播的主要途径。Objective To detect hepatitis C virus (HCV) RNA in amniotic fluid of gravida and investigate mother-to-infant transmission of HCV. Methods Thirty-four HCV seropositive gravida (experimental group) were engaged. Fluorescence quantitative polymerase chain reaction (PCR) based on amplisensor assay and reverse transcription -PCR (RT-nPCR) was used. Serum HCV RNA positive sera were genotyped by RFLP analysis of PCR products from 5′NC region. Sera and amniotic fluid samples of 40 normal gravida were set as the control group. Results In the experimental group, HCV RNA was detected in amniotic fluid (5.9%, 2/34) of 2 cases. HCV RNA titers were 10 5 and 10 6 copy/ml respectively. No HCV RNA was detected in the amniotic fluid and sera of the control (n=40). Conclusions HCV RNA was rarely detected in amniotic fluid. The amniotic fluid is not the main route of HCV mother-to-infant transmission.
