股骨髁部骨折内固定方式的选择及临床应用

摘　　要：目的 :总结各种常用内固定方式的效果。方法 :临床应用不同内固定方式治疗股骨髁部骨折 16 5例 ,其中 12 8例得到随访 ,平均随访时间为 2 5年。结果 :优良率分别为 :动力加压髁螺钉 (DynamicCondylarScrew ,DCS) 85 71%、T型钢板加压松质骨螺钉 85 %、L型钢板 83 33%、加压松质骨螺钉 82 14%。结论 :①松质骨加压螺钉适合于B型股骨髁部骨折 ,尤其适用于骨折块较大、骨质较好的青壮年患者。②DCS适合于股骨髁上、髁间骨折的固定 ,但不适合于近关节面处骨折。③L型钢板和T型钢板加松质骨加压螺钉效果相近 ,但T型钢板加松质骨加压螺钉固定操作简单、损伤小。Objective:To provide clinical internal fixation method choice.Methods:To sum up the results of clinical application of different internal fixation methods in Among 165 femur condylar fracture cases.Results:Among 165 postoperation femur condylar fracture cases,128 cases were followed up with average time of 2.5 years,in which the good rates of dynamic condylar screw,T-type plate add cancellous compression screw,L-type plate,cancellous compression screw were 85.71%,85%,83.33%,82.14%.Conclusion:Cancellous compression screw fixation is a simple and effective internal fixation method for B and C-type fracture of femur condylar.
