摘 要:目的 调查我院产超广谱β-内酰胺酶 ( ESBL s)的肠杆菌科细菌的流行情况和耐药现状。 方法 用VITEK3 2全自动细菌分析仪和 GNS- 5 0 6药敏卡进行 ESBL s检测和药敏试验。 结果 在 2 72株肠杆菌科细菌中 ,共检出产 ESBL s的大肠埃希菌、肺炎克雷伯菌、阴沟肠杆菌、产气肠杆菌、弗劳地枸橼酸杆菌、产酸克雷伯菌、阿斯布肠杆菌和臭鼻克雷伯菌 3 8株 ,总检出率为 13 .97% ,以重症监护病房 ( ICU )的分离率为最高 ( 3 3 .3 3 % ) ;产 ESBL s菌对大多数抗生素的耐药率显著高于非产 ESBL s菌 ( P<0 .0 1) ,且多重耐药性显著 ,但对亚胺培南均敏感。 结论 肠杆菌科细菌中产ESBL s的细菌种类较多 ,耐药性严重 ,临床实验室应重视对肠杆菌科细菌进行 ESBL s的常规检测。Objective To investiagte the epidemiological and antibiotic resistance of clinical isolates of the family Enterobacteriaceae that produce extended-spectrum β-lactamases(ESBLs)in our hospital. Methods Vitek GNS-506 was used to detect ESBLs and antibiotic sensitivity. Results 13.97% of 272 strains of the family,Enterobacteriaceae were considered ESBLs producers including E.cdi,K.pneumoniae ,K.oxytoca,K.ozaenae,E.cloacae,E.areogenes,E.asburiae and C.freundii, ICU was the most epidemical ward with ESBLs(33.33%);The resistance rate of ESBLs-producing strains against multiple antibiotics were significantly higher than those of non-ESBLs producing strains(P<0 01),while the multiresistant characteristics of ESBLs-producing strains were remarkable too.But all of them were susceptible to imipenem. Conclusions The results indicated that ESBLs-producing members of the family Enterobacteriaceae were more and more and the antibiotic resistance of them was serious.It is necessary for clinical laboratory to carry out routine ESBLs detection in clinical isolates of the family Enterobacteriaceae .
