益母草制剂治疗心肌缺血及其机制研究

摘　　要：目的 :观察益母草注射液 (或片剂 )对动物及冠心病患者心肌缺血的治疗效果并探讨其机制。方法 :30只新西兰白兔随机等分为正常组、缺血组和益母草注射液治疗组。5 2例冠心病及 30例无症状性心肌缺血患者静滴益母草注射液 ;5 4例心肌缺血患者口服益母草片。结果 :治疗组兔左室收缩内压 (L VSP)和最大变化速率 (± dp/ dtm ax)均高于缺血组 ,左室舒张内压 (L VEDP)、ΣST段则明显低于缺血组 (P<0 .0 5或 P<0 .0 0 1) ;血浆肌酸激酶、天冬氨酸转氨酶、乳酸脱氢酶、α 羟基丁酸脱氢酶活性及丙二醛含量治疗组均明显低于缺血组(P均 <0 .0 1) ,而全血谷胱甘肽过氧化物酶活性治疗组则高于缺血组 (P<0 .0 5 )。冠心病患者经益母草注射液治疗后 ,症状、体征、心电图、血脂、微循环及血液流变学指标均明显改善 ;心肌缺血患者用益母草片治疗 30日 ,心肌缺血症状、心电图、血脂、血液流变学指标均明显改善。结论 :益母草制剂对心肌缺血有明显疗效 ,其机制与减少氧自由基对心肌的损伤 ,改善缺血区心肌微循环及血液流变性有关。Objective:To investigate the curative effect of herba leonuri on myocardial ischemia in rabbits and patients and its mechanism.Methods:Thirty New Zealand rabbits were randomly divided into three groups:the normal control group (NCG),the ischemia control group (ICG) and the herba leonuri treatment group (HLG).After 20 minutes of ischemia,7 5 g/kg leonuri in 5% glucose 30 ml was transfused in HLG and 5% glucose in NCG as well as ICG.Also,52 cases with myocardial ischemia (MI) and 30 cases with silent myocardial ischemia (SMI) were treated with injection leonuri 15 ml in 5% glucose 250 ml once a day.The course of treatment was 15 days.Fiftyfour cases with myocardial ischemia were treated with tablet herba leonuri for 30 days.Results:The cardiac functions of HLG were improved,and values of left ventricular systolic pressure(LVSP),±dp/dtmax in HLG were obviously higher that those in ICG ( P <0 05 or P <0 001),the values of left ventricular enddiastolic pressure(LVEDP) and ΣST segment voltage in HLG were obviously lower than those in ICG ( P <0 05 or P <0 001).The plasma activities of creatine kinase(CK),aspartate amino transferase(AST),lactate dehydrogenaseL(LDHL),αhydroxybutyrate dehydrogenase(αHBDH) and malondialdehyde(MDA) levels in HLG were significantly lower than those in ICG (all P <0 01),while the activities of glutathione peroxidase(GSHPx) in blood was obviously higher than that in ICG ( P <0 05).On the other hand,15 days after herba leonuri injection,the symptoms and sign,electrocardiogram(ECG),blood lipid,indexes of hemorrheology and microcirculation in MI and SMI patients were obviously improved ( P <0 05 or P <0 01).After 30 days of tablet herba leonuri treatment in MI patients,the symptoms of myocardial ischemia,ECG,lipidemia,hemorrheology were obviously ameliorated ( P <0 05 or P <0 01).Conclusions:The results indicated that herba leonuri appears to be effective in curing myocardial ischemia.Its mechanism may be related to allaying myocardial injury induced by oxy
