胎儿先天性心脏病的超声筛选分析

摘　　要：目的　探讨超声在筛选胎儿心血管异常中的应用价值。方法　采用彩色多普勒超声检查 7166例16～ 42周胎儿心脏 ,产前超声诊断的复杂心内畸形与胎儿尸解或分娩后超声复查对照。结果　①产前超声发现46例胎儿心血管异常 ,异常率为 6.42‰ ,其中复杂性先心病 10例经引产后尸解证实。复杂性先心病多合并体表畸形或多系统畸形。②单纯室间隔缺损 2 0例 ,其中 10例因合并其他畸形引产尸解证实 ,4例产后证实 ,6例室间隔缺损 <5mm者为超声漏诊。③单纯房间隔缺损 6例 ,其中 4例合并巴氏水肿及多系统畸形 ,引产证实。④卵圆孔过大 10例 ,2例产后证实为房间隔缺损 ,8例正常。⑤超声发现心包积液 8例 ,心律失常 9例。结论　胎儿心脏异常并非少见 。Objective To assess the value of ultrasonography in diagnosing abnormality of fetus.Methods Seven thousand and one hundred sixty six fetus range from 16～42 weeks gestation were detected by color Doppler ultrasound,and the results were compared with post parturition re examination and fetal autopsy.Results ①There were forty six cases of fetal cardiovascular abnormality,whose abnormality rate was 6.42‰ .Ten cases of whom were confirmed by autopsy after induced labor and they always complicated surface and multiple system malformation.② Among twenty cases of simple ventricular septal defect (VSD) were found,and ten cases were confirmed by autopsy after induced labor because of complicating other system malformation,four cases were confirmed post delivery,six cases with VSD diameter <5 mm were missed diagnosis.③ Among six cases of simple atria septal defect (ASD) were found,and four cases were confirmed complicated with Bart'edema and multiple system malformation after induced labor.④ Among ten cases of fossa oval enlargement were included,two cases were confirmed to be ASD post delivery,and eight cases were normal.⑤ Pericardial effusion was found in eight cases,and nine cases with cardiac arrhythmia.Conclusions There are not few of abnormal fetal hearts and only correct diagnosis is favorable to improve baby quality.
