摘 要:观察不同剂量甘露醇对兔肾脏功能、肾血流量及一氧化氮 (NO)生成的影响 ,探讨甘露醇诱导急性肾功能衰竭 (ARF)的可能机制。 18只健康新西兰大白兔随机分为三组 ,分别给予小剂量甘露醇、生理盐水、大剂量甘露醇并自由饮水 ,观察肾脏功能、肾血流量及血和尿中NO代谢终产物NO-2 /NO-3 的变化。小剂量甘露醇组肾功能、肾血流量及血NO-2 /NO-3 与对照组比较差异均不显著 (P >0 0 5 ) ;而大剂量并自由饮水组肾功能、肾血流量明显下降 ,血NO-2 /NO-3 明显上升 ,与对照组及小剂量组比较差异有显著性 (P >0 0 5 )。三组间尿NO-2 /NO-3 排泄速率差异无显著性 (P >0 0 5 )。提示大剂量甘露醇诱导急性肾功能衰竭与其明显降低肾血流量有关 ,而内皮衍化舒张因子 (EDRF)生成似有代偿性增高。To observe the changes of renal blood flow and serum NO - 2/NO - 3 in rabbits administrated with mannitol at different dosages, and investigate the mechanism of acute renal failure (ARF) induced by massive mannitol. Eighteen healthy male Zealand rabbits were randomly devided into 3 groups:minor mannitol group(n=6, 8g·kg -1 within 2 hours), control group (n=6, saline of the same volume),massive mannitol group with free water taking(n=6, mannitol 40~60g·kg -1 with 3 days). The changes of renal function, renal blood flow and NO - 2/NO - 3 in serum and urine were observed. The results showed that: no significant change in creatinine clearance (Ccr), renal blood flow (RBF) and serum NO - 2/NO - 3 was found between minor mannitol group and control group. In massive mannitol group with free water taking, Ccr and RBF significantly decreased and serum NO - 2/NO - 3 significantly increased (P>0.05). The results suggest that ARF induced by massive mannitol is associated with a significant decline of RBF, on the other hand, endothelium derived relaxing factor(EDRF) maybe compensatively increase.
