摘 要:目的 探讨心房晚电位测试对于预测阵发性心房颤动发生的临床关联。方法 以P波触发体表信号平均心电图记录技术测试 47例阵发性心房颤动患者 (试验组 )及 40例无症状者 (对照组 )的心房晚电位 (包括滤波后P波时限 ,P波终末 2 0ms、30ms、40ms的电压均方根值 )并进行比较分析。结果 试验组的P波终末2 0ms电压均方根值为 (2 .6 6± 0 .2 6 ) μv ,对照组为 (3.73± 0 .2 0 ) μv ,两组比较有显著性差异 (P <0 .0 0 1) ;试验组的滤波后P波时限为 (136 .6 2± 2 .91)ms ,对照组为 (114.0 5± 1.18)ms,两组比较有显著性差异 (P <0 .0 0 1) ;试验组的P波终末 30ms电压均方根值为 (3.6 8± 0 .30 ) μv ,对照组为 (4.73± 0 .2 6 ) μv ,两组比较有显著性差异 (P<0 .0 2 ) ;结论 P波触发体表信号平均心电图记录心房晚电位可作为预测阵发性房颤的一个快速、无创性指标。Objective To investigate the relationship between atrial late potential (ALP) and paroxysmal atrial fibrillation(PAF). Methods Forty seven patients with PAF (AF group) and 40 normal individuals (normal group) had their ALP measured with the P waves triggered signal averaged electrocardiogram and comparatively analyzed. The measurements included mean root square (MRS) voltages for the last 20, 30, and 40 ms(LP 20 , LP 30 ,and LP 40 ) and the duration of filtered (40 250 Hz) P wave of the magnitude (Ad). Results LP 20 was significantly lower in AF group than that in normal group (2.66±0.26 vs 3.73±0.20μv, p <0.001); Ad was significantly higher in AF group than that in normal group (136.62±2.91 vs 114.05±1.18 ms, p <0.001); and also significant difference in LP 30 was found between the two groups (LP 30 : 3.68±0.30 vs 4.73±0.26μv, p <0.02). Conclusion ALP measurement with P waves triggered signal averaged electrocardiogram can be a rapid and non traumatic index to predict paroxysmal atrial fibrillation.
