体外超声抽脂术及其脂肪乳化效应

摘　　要：目的　探讨肿胀麻醉体外超声在抽脂术中的作用与疗效。方法　对 5 0例肥胖患者应用肿胀麻醉体外超声抽脂技术进行腹部、大腿等部位脂肪抽吸。取 8例抽吸物标本行HE染色、切片、10× 10倍光镜下观察脂肪细胞形态学变化。结果　 5 0例肥胖患者均取得满意的手术效果 ,术后并发症为皮肤局部青紫瘀斑、感觉迟钝 ,吸脂部位凹凸不平等 ,均自行恢复 ,术后 3个月随访 ,满意率达 98%。 8例标本病理切片观察 :抽吸物细胞间连接较完整 ,细胞破裂的比率为 5 %～ 15 % ,未见明显脂肪细胞乳化效应。结论　本研究提示体外 3W /cm2 、1MHz超声能量不能产生明显脂肪细胞乳化效应 ,在抽脂术中可能起辅助协同作用 ,肿胀技术在提高疗效、减少并发症上起主要作用。Objective To explore the clinical efficacy of external ultrasound liposuction combined with tumescent anesthesia. Methods Fifty cases were asked to accept the treatment of external ultrasound assisted-lipectomy. Eight separate specimens from aspirating underwent fixation and HE staining for optical microscopy. Results 98 percent cases were satisfied with their surgical results by following up within 3 months, only minor complications such as irrigularities. Sense bluntness and bluise were gotten and restored spontaneously. 8 separate specimens showed only 5～15 percent of cellular disruption; intercellular juction integrity and no apparent fat cell emulsification. Conclusion External ultrasound may be an adjunct form in liposuction, which cannot emulsificate fat tissue, while tumescent techenique plays a major role in improving clinical efficacy. External ultrasound liposuction combined with tumescent techenique is a safe and effective method of lipoplasty.
