胸腺切除治疗重症肌无力的围手术期护理

摘　　要：目的　探讨胸腺切除治疗重症肌无力 (MG)的围手术期护理的必要性。方法　胸腺切除是治疗重症肌无力的有效手段 ,术前应用激素和抗胆碱酯酶药物控制肌无力症状 ,为手术创造条件 ,巩固疗效。术前后主动进行心理护理 ,减少重症肌无力危象 (MGC)发生。MG患者术后给予重症监护并根据病情留置气管插管 ,应用呼吸机进行人工辅助呼吸。加强呼吸道管理 ,保持呼吸道通畅 ,防止肺部感染是预防和抢救MGC的关键。应用肠内肠外营养支持。结果　本组 6 6例手术病人采用此围手术期的治疗与护理方法 ,无手术及住院死亡 ,全部按期出院。结论　围手术期护理 。Objective To probe the necessity of postoperative and preoperative cares of myasthenia grasis(MG) treated by thymusectomy. Methods Thymusectomy is an effective way to treated MG. Before operation, with hormone and anti cholinesterase drugs to control symptoms, with mental cares to decrease myasthenia grasis crisis occurrence. After operation. the patients were given severe monitor with retaining tracheal cannula, were assisted respiration with respirator. Enhancing respiratory tract management, keeping from obstruction, preventing from lung infection were the keys of emergency treatment and prevention. Intra intestinal and abenteric dietotherapy can promote the recovery. Results With effective postoperative and preoperative cares, there were no death, all cases discharged on times. Conclusions Postoperative and preoperative cares are the base of the operation success.
