摘  要:目的:检验在多导睡眠监测的同时使用压力监测系统对阻塞性睡眠呼吸暂停综合征患者睡眠结构的影响。方法:2000年11月至2001年1月,随机在同仁医院睡眠监测中心选择可疑睡眠呼吸暂停综合征的患者120例,分为年龄、体重指数等方面具有可比性的两组,86例对照组单纯进行多导睡眠呼吸监测,34例试验组同时加用压力监测系统测量食管下1/3和咽部的压力变化。对比两组患者受检查当夜的睡眠结构。结果:试验组的患者与对照组相比,Ⅲ和Ⅳ期睡眠占总睡眠时间的百分比[非正态分布,用中位数加范围描述0.6(0.0~71.0)% vs 0.9(0.0~38.0)%,秩和检验P=0.387]、快动眼睡眠占总睡眠时间的百分比(7.6±4.4% vs 9.5±7.8%,P=0.105)和睡眠有效率(87.9±8.7% vs 89.4±8.8%,P=0.380)无显著差异。两组睡眠的差别,仅存在于入睡潜伏期上[非正态分布,用中位数加范围描述16.3(0.0~100.5)分钟 vs 8.0(0.0~94.5)分钟,秩和检验P=0.011],但两组相差量极小,不影响睡眠有效率。结论:压力监测系统的使用对睡眠结构影响轻微,患者对测压系统耐受性很好。压力?Objective:To identify the affects of using esophageal pressure measurement system(manometry system)on the sleep architecture of OSAS(obstruct sleep apnea syndrome) patients during polysomnographic examination at night.Methods:From Nov.2000 to Jan 2001,one hundred and twenty patients with OSAS in Tongren Hospital were selected randomly which were divided into two groups.The control group(86 cases)only underwent the polysomnographic examination ,subject group(34 cases)was also monitered the pressure at the lower part of esophagus and pharynx with manometry system si multaneously.Results:There were not differences between subject group and control group in age(44.2±10.9 vs 43.4±11.7,p=0.759),sex(Male/Female:29/5 vs 73/13,p =0.995),body mass index(27.9±4.3kg/m2 vs 28.1±3.9kg/m2,P=0.869)and apnea-hypopnea index(52.6±30.0 vs 45.6±30.7,P=0.260).No difference was found between two groups in sleep effect(87.9±8.7% vs 89.4±8.8%,P=0.380),percentage of Ⅲ a nd Ⅳ stage sleep [abnormal distribution,described by median and range 0.6%(0.0~71.0%) vs 0.9%(0.0~38.0%),rank sum test P=0.387]and Rapid Eye Movement(REM)sta ge sleep(7.6±4.4% vs 9.5±7.8%,P=0.105).The only difference was found in latenc y to persistent sleep between two groups [abnormal distribution,described by med ian and range 16.3minute(0.0~100.5) vs 8.0(0.0~94.5),rank sum test P=0.011].Conclusio n: Sleep architecture was slightly disturbed by using manometry system with poly somnographic examination simultaneously.Only the latency to persistent sleep had significant difference in statistics but was too little to be considered in clinical aspect.Manometry could be a routine examination for sleep apnea syndrome.\= [Chinese Arch Otolaryngol Head Neck Surg,2002;9(1):27~30 From the Department of Otolaryngology-Head and Neck Surgery,Affiliated Beijing Tongren Hospital of Capital University of Medical Sciences,Beijing 100730(Dr.Chen Xuejun)]
