上睑沟凹陷及结膜囊畸形的整复

摘　　要：目的　探讨眼球摘除术后上睑沟凹陷的手术方法的改良及其材料的选择。方法　轻型上睑沟凹陷采用重睑手术皮肤切口 ,行眼轮匝肌下潜行分离 ,植入自体、异体生物材料或硅胶海绵。重型上睑沟凹陷采用不同大小、形状、材料的义眼台二次植入 ,结膜囊成形及上睑沟充填 ,解决义眼台在眼内移位、结膜囊畸形及上睑沟凹陷的矫治问题。结果　 6 6例 (轻型 2 8例 ,重型 38例 )术后外观满意 ,义眼活动度好 ,改善了义眼后陷及上睑沟凹陷 ,观察 3～ 5年 ,未见严重并发症。结论　轻型上睑凹陷的手术矫正 ,采用改良的方法 ,效果满意。重型上睑沟凹植入义眼台 ,补充丢失的眶内容 ,顶起上直肌上睑提肌复合体 ,使二次义眼台植入 。Objective To study the new surgical method and new materials for correcting the deepened superior sucus and conjunctival sac deformity after enucleation.Methods For simple deepened superior sulcus deformity,a piece of elastic sponge was used to fill the space of subocular muscle through a transverse incision paralleling the upper palpebral limber.For complicated conjunctival sac deformity,the migrated orbital implant was removed and replaced by the new designed magnetic hydroxyapatite sphere or conical implant with superior projection.Results In 66 cases,simple deepened superior sulcus deformity were 28 cases,complicated conjunctival sac deformity were 38 cases.The deformity of all cases have been corrected with improved prosthetic motility and appearance.Conclusion The reform of surgical incision and implant's materials for simple deepened superior sulcus deformity brought about a good cosmetic effect.It not only extend the operative field but also simplify the operation and make the implant fixed steady after the operation.The new designed intraorbital implants can compensate the orbital content,project the superior sulcus,enhance the prosthetic motility and improve the appearance.
