中重度原发性下肢深静脉瓣膜功能不全戴戒术的疗效分析

摘　　要：目的 探讨股浅静脉戴戒术治疗中重度原发性下肢深静脉瓣膜功能不全的疗效。方法 对５４例原发性下肢深静脉瓣膜功能不全患者进行下肢深静脉顺逆行造影，提示倒流程度均为２～４级，通过分析，将其分为深静脉显著增粗但瓣膜瓣叶的游离缘并未严重受损及瓣膜瓣叶的游离缘严重受损２类，并对深静脉显著增粗但瓣膜瓣叶的游离缘并未严重受损３２例进行股浅静脉戴戒术。结果 疗效良好２９例、占９０．６％，好转者３例、占９．４％，全部患者症状消失，溃疡愈合，多普勒超声示静脉无明显血液倒流。结论 股浅静脉戴戒术是治疗瓣膜瓣叶未严重受损的中重度原发性下肢深静脉瓣膜功能不全的有效方法，静脉造影是判断手术适应证的重要依据。Objective To discuss the curative effect of severe primary deep venous valve incompetence in the lower limbs by ringing of superficial femoral vein.Methods The 54 patients with primary deep venous incompetence valve underwent direct and antidromic venous angiography on lower limbs,the angiography showed that reflux degree was 2-4 grade.The 32 out of 54 patients without serious injured dissociative edge of valve leaves were treated by ringing of superficial femoral vein.Results The excellent curative effect was 29(90.6%), a turn for better was 3(9.4%).The symptoms disappear and ulcers were healed in all the patients.The Doppler ultrasonography showed that blood was not distinct flow backwards in the vein.Conclusion Ringing of superficial femoral vein is an effective method in treatment of severe primary deep venous valve incompetence without serious injured dissociative edge of valve leaves.The venous angiography is important basis of determining operative type.
