摘  要:目的探讨血管内皮生长因子 (VEGF)在门静脉高压症断流术后食管静脉曲张再生中的作用。方法制备肝前型门静脉高压症 (PHPH)大鼠模型 ,并设立假手术 (SO)对照。在术后 3周施行断流术 ,分别在断流术前及术后 1、3、5、7、14、30、90d取材。用免疫组织化学法检测第八因子相关抗原 (FⅧAg)、VEGF在食管组织中的表达。对FⅧAg阳性血管进行微血管密度计数 (MVD) ,对VEGF染色结果进行计算机图像定量分析。结果PHPH组大鼠在断流术前MVD较SO组显著增高(P <0 0 1) ,在断流术后则较术前明显下降 (P <0 0 1) ,但在术后 5d又开始增高 ,到术后 1个月时与断流术前差异无显著意义 (P >0 0 5 ) ,至术后 3个月时 ,较SO组显著增高 (P <0 0 1) ;SO组在断流术后未见MVD的下降 ,而是在术后 3d达到高峰 (P <0 0 1) ,以后基本维持在此水平。PHPH组在断流术前VEGF表达显著高于SO组 (P <0 0 1) ,2组均在断流术后出现VEGF表达高峰 ,但PHPH组表达时相较SO组显著延长 (P <0 0 1)。Objective To evaluate the effect of vascular endothelial growth factor (VEGF) expression on varicose recurrence in esophagus after devascularization in portal hypertensive rats. Methods Murine pre-hepatic portal hypertension (PHPH) was induced by partial portal vein stenosis. 3 weeks after the establishment of the PHPH, all rats including sham operation group (SO) underwent devascularization. Esophagus was sampled at the time of 1, 3, 5, 7, 14, 30, 90 days after devascularization respectively. The expression of VEGF and factor Ⅷ related antigen (FⅧAg) was detected by immunohistochemistry,and the microvessel density (MVD) was counted under FⅧAg staining. ResultsCompared to the SO rats, the MVD of esophagus in PHPH rats increased significantly before devascularization(P<0.01), but decreased immediately after the operation(P<0.01), then increased gradually from the fifth day until it reached the pre-operative level one month after(P>0.05). At the third month after devascularization, the MVD of PHPH rats was higher than that of SO rats. In SO rats, MVD increased after devascularization, reaching the peak on the third day and then levelling off.The expression of VEGF before devascularization in PHPH rats was significantly higher than in SO rats, and VEGF experssion was on the pinnacle in two groups after devascularization, but the expression time of PHPH rats was significantly longer than SO rats(P<0.01). Conclusion The increased expression of VEGF contributes to the regeneration of esophageal varices after devascularization in portal hypertension.
