胫后血管间隙支胫骨内侧骨膜瓣移位术的临床应用

摘　　要：目的　探讨应用带血供 (胫后血管肌间隙支 )胫骨内侧骨膜瓣移位术治疗胫骨中下段骨折及骨不连。方法　 2 3例胫骨中下段骨折及骨不连患者均采用切取该骨膜瓣移位于骨折处。结果　 2 3例患者经切开复位固定、该骨膜瓣移位术治疗 ,术后骨折部血供良好 ,骨折一期愈合 ,未发生骨折延迟愈合或骨不连现象。患肢功能恢复满意。结论　由于该骨膜瓣较厚 ,血运丰富 ,供体量大 ,能够较好的促进骨的愈合。特别对于骨延迟愈合、骨不连发生率较高的胫骨中下 1/ 3骨折 。Objective To describe a method of treatment of nonunion of fracture of tibia over the interior medium segment with periosteum flap transposition Methods 23 cases of medium inferior tibial fracture and nonunion were treated with the transposition of medial tibial periosteum flap pedicled with intermuscular branch of posterior tibial vessels.Results 23 cases were treated with open reduction,fixation,and transposition of the periosteum flap.The fracture had ample blood supply from the pedicled flap after operation and healed up by first intention.The affected extremity had satisfactory functional recovery.Conclusion The pedicled flap is able to promote bony fusion with advantages of ample blood supply and large area of coverage,especially in cases of medium inferior segmental fractures of the tibia.
