摘  要:目的 使用许勒位片及数字减影技术对髁突移位进行定量分析 ,并评价其准确性及临床应用。方法颞下颌关节弹响者 8例 ,均行复位牙合垫治疗并拍摄治疗前后的许勒位片 ,选择 8例志愿者 (16侧关节 )间隔 3天~ 7天重复拍摄许勒位片 ,使用数字减影法测量治疗前后髁突水平向及垂直向的移位值 ,并测量髁突与关节窝距离的差值。 8例志愿者进行许勒位重复性检验。结果 戴牙合垫前后关节间隙线性测量 :治疗前关节上、后、前间隙的均值分别为 2 .5mm、1.9mm、2 .3mm ;戴牙合垫后髁突明显前下移位 ,关节上、后、前间隙的均值分别为 4.0mm、5 .0mm、2 .3mm ;数字减影法髁突水平向移位均值为 0 .46mm ,垂直向移位均值为 0 .5 9mm。结论 许勒位片数字减影可用于髁突移位的定量分析。Objective Condyle positional change on transcranial images was measured by means of digital subtraction and its validity and clinical application were evaluated.Methods 8 patients with temporomandibular disorders and 8 volunteers were included in the study.Transcranial images were taken.The amount and direction of condyle displacement were assessed on digitally subtracted image with and without splint.Joints space was measured with and without splint.Results The mean values of condyle displacement on horizontal and vertical direction were 0.46 mm, 0.59 mm respectively.Measured superior,posterior and anterior joint spaces were 2.5 mm, 1.8 mm, 2.3 mm and 4.0 mm, 5.0 mm, 2.3 mm with and without splint.Conclusion Digital subtracted transcranial image can be used in measurement of condyle displacement.
