10毫克米非司酮在治疗子宫肌瘤中的应用

摘　　要：目的 探讨对子宫肌瘤患者应用不同剂量米非司酮的疗效.方法 选取我院2015年2月~2016年8月收治的子宫肌瘤患者150例,根据米非司酮使用剂量的不同分为10 mg组(80例)和25 mg组(70例).治疗3个月后,比较组间血清激素指标变化、子宫及子宫肌瘤改善情况、月经变化、不良反应.结果 ①治疗前,两组月经变化、子宫体积和肌瘤体积差异无统计学意义(P>0.05),治疗1个月及治疗3个月后,两组月经变化、子宫体积及肌瘤体积差异均有统计学意义(P<0.05),对应时间节点,组间差异无统计学意义(P>0.05);②治疗前,两组血清激素T、PRL、LH、FSH、E2及P差异无统计学意义(P>0.05).治疗1个月及3个月后,两组T、PRL比治疗前及组间相比差异无统计学意义(P>0.05),LH、FSH、E2和 P均显著下降(P<0.05),组间差异无统计学意义(P>0.05);③用药3个月内,10 mg组不良反应率(31.25%)显著低于25 mg组(51.43%)(P<0.05).结论 日服10 mg米司非酮治疗子宫肌瘤可显著改善患者血清激素表达水平、缩小子宫及子宫肌瘤体积,不良反应较25 mg更少,月经较为规则、稳定.Objective To investigate effects of different doses of mifepristone in the treatment of uterine fibroids.Methods 150patients with uterine fibroids treated in our hospital from Feb.2015to Aug.2016were selected.Patients were divided into 10mg group (80cases)and 25mg group (70cases)according to the dosage of mifepristone.Hormone indexes,improvement of uterus and uterine fibroids,menstrual changes and adverse reactions were com- pared between the groups after 3months'treatment.Results①There were no significant differences on menstrual changes,volume of uterus and uterine fibroids between the two groups before treatment(P >0.05).There were significant differences on menstrual changes,volume of uterus and uterine fibroids between the two groups after one month' s and three months'treatment than those before treatment(P <0.05).There were no significant differences between the two groups at corresponding time nodes (P >0.05).②There were no significant differences on T,PRL,LH,FSH, E2and P between the two groups before treatment (P >0.05).There were no significant differences on T and PRL between the two groups after one month's and three months'treatment than those before treatment(P >0.05).LH, FSH,E2and P decreased after one month's and three months'treatment than those before treatment (P <0.05). There were no significant differences between the two groups at corresponding time nodes (P >0.05).③The rate of adverse reactions in 10mg group (31.25%)was significantly lower than that in 25mg group (51.43%)within medication for 3months (P <0.05).Conclusion 10mg oral mifepristone per day for the treatment of uterine fibroids can significantly improve serum hormone levels,reduce the volume of uterus and uterine fibroids,which has less adverse reaction than 25mg oral mifepristone and the menstruation is regular and stable.
