摘  要:目的探讨2型糖尿病患者颈动脉斑块积分(PS)与25羟维生素D_3[25(OH) D_3]的关系。方法测定并记录138例2型糖尿病患者的一般临床资料及实验室检查结果,同时测定双侧颈动脉内膜中层厚度及斑块情况,将双侧颈动脉所有斑块的最大厚度之和定义为PS。按照有无斑块及PS的中位数,将2型糖尿病患者分为3组,第一组PS=0,即无斑块组(PS1组) 40例,第二组0 <PS≤4. 6(PS2组) 50例,第三组PS>4. 6(PS3组) 48例。比较3组患者的一般临床资料及检测结果的差异。结果 PS1组、PS2组及PS3组患者的年龄、25(OH) D_3及收缩压差异有统计学意义(P <0. 05),其中25(OH) D_3呈现逐渐减低的趋势,且任意两组间25(OH) D_3的水平差异有统计学意义(P <0. 05)。通过相关性分析发现年龄及收缩压分别与PS呈显著正相关(r=0. 340,P=0. 001; r=0. 287,P=0. 004),25(OH) D_3与PS呈显著负相关(r=-0. 313,P=0. 002)。多元线性逐步回归分析分析发现,25(OH) D_3与PS呈独立的负相关(β=-0. 014,P=0. 001),年龄与PS呈现独立的正相关(β=0. 009,P <0. 001)。结论 PS与25(OH) D_3呈现独立的负相关,随着25(OH)D_3水平的下降,PS可能会呈现增大的趋势。Objective To investigate the correlation between carotid plaque score(PS)and25(OH)D3among type2diabetic patients.MethodsA total of138type2diabetic patients were enrolled in this Study.General clinical data and laboratory examination results were measured and recorded.And bilateral carotid intima-media thickness (IMT)and carotid plaques were also measured.Summing up the maximum thickness of all plaques located in bilateral ca- rotid arteries was defined as PS.According to whether there was a plaque or not and the median of PS,type2diabetic pa- tients were divided into3groups.The differences of general clinical data and examination results among3groups were compared.ResultsThere were significant differences in age,25(OH)D3,and systolic pressure among the3groups (P<0.05).Significant reduction trend was observed in25(OH)D3(P<0.05).Age and systolic pressure were signifi- cantly positively correlated with PS(r=0.340,P=0.001;r=0.287,P=0.004),and25(OH)D3was significantly negatively correlated with PS(r=-0.313,P=0.002).Multiple linear stepwise regression analysis showed that25 (OH)D3was independently negatively associated with PS(β=-0.014,P=0.001),and age was independently posi- tively associated with PS(β=0.009,P<0.001).ConclusionThe25(OH)D3is an independent protective factor a- gainst PS.
