椎弓根钉棒系统治疗骨盆骨折九例

摘　　要：目的评价椎弓根钉棒系统治疗骨盆骨折的近期疗效。方法 2014年1月至2017年12月攀枝花市中心医院骨科应用椎弓根钉棒内固定治疗骨盆骨折9例,男性7例,女性2例;年龄25～66岁,平均44.8岁;采用Tile分型,其中B1型2例,B2型2例,B3型1例,Tile C1型2例,Tile C2型2例。B型骨盆骨折直接采用椎弓根钉棒内固定术,C型骨盆骨折先固定后环,再采用椎弓根钉棒系统固定前环。术后采用Matta评分评价临床效果。结果术后所有患者均获得随访,骨折均愈合,随访时间6～12个月,平均8.5个月,1例患者出现股神经麻痹,5例患者出现股外侧皮神经损害表现,4例患者坐位时出现皮肤激惹。采用Matta评分,其中优3例,良3例,可2例,差1例。结论椎弓根钉棒系统治疗骨盆骨折可出现较多的并发症,近期效果并不十分令人满意,其应用应掌握相应的适应证,且医师需对骨盆骨折的治疗有丰富的临床经验。Objective To evaluate the short-term curative effect of pedicle screw and rod system in the treatment of pelvic fractures. Methods From January 2014 to December 2017, 9 patients(including 7 males and 2 females)with pelvic fractures were treated with pedicle screw and rod system for internal fixation in Department of Orthopedics,Panzhihua Central Hospital. The average age of the patients was 44.8 years old(ranged from 25 to 66 years old). According to Tile classification, there were 2 patients of type B1, 2 cases of type B2, 1 case of type B3, 2 cases of type C1, 2 casesof type C2. Type B pelvic fractures were treated directly with pedicle screw and rod system. Type C pelvic fractures were fixed first with the posterior ring, and then with the pedicle screw and rod system for the anterior ring fixation.Matta score was used to evaluate the clinical effect after operation. Results All the patients were followed up and the fracture was healed. The follow-up time was 6 to 12 months(8.5 months in average). One patient showed femoral nerve paralysis, five patients had lateral femoral cutaneous nerve damage, and four patients had skin irritation in sitting position. According to Matta’s score: there were 3 cases of excellent, 3 cases of good, 2 cases of fair and 1 case of poor. Conclusion A lot of complications can occur in the treatment of pelvic fractures with pedicle screw and rod system.The short-term results are not satisfactory.Its application should based on corresponding indications, and doctors should have rich clinical experience in the treatment of pelvic fractures.
