摘  要:目的检测儿童肛门生殖器疣的HPV病毒基因型。方法门诊收集12岁以下的儿童肛门生殖器疣患者,记录病史及查体,留取皮损处脱落细胞标本,运用PCR-反向点杂交法检测15种高危型别,3种疑似高危型别,10种低危型别和1种皮肤型别。统计患者的一般情况、皮损分布和HPV检查结果。结果共收集了45例4个月~12岁的儿童肛门生殖器疣患者,男女分别为12例,23例,男女比例0. 4∶1,年龄中位值4岁,病程4个月。皮损分布在肛周者最多为38例(84. 44%),其次外生殖器8例(17. 78%)。8例(17. 78%)为多部位发病,其余37例(82. 22%)均为单部位发病,单发于肛周者最多为30例(66. 67%)。HPV检测阳性者有24例,阳性率53. 33%,6型感染16例(66. 67%),11型感染4例(16. 67%),7型感染4例(16. 67%),59+6型混合感染1例。HPV6型在性别间分布差异有统计学意义(P=0. 023)。结论本地区儿童肛门生殖器疣HPV感染亚型以低危型6型为主,多为单一型感染,皮肤型感染的比率可能被低估。Objective To detect the HPV genotypes in children with anal and genital warts.Methods Investigated the medical history,took physical examination in patients under 12 years with ano-genital warts then collected the exfoliated cells on lesions.PCR-reverse dot blot method was adopted to detect the HPV genotype including 15 high-risk viruses,3 suspected high-risk virus,10 low-risk virus and 1 cutaneous type.The general condition was described,skin lesion distribution and HPV test were analyzed between genders. Results A total of 45 child were enrolled,the average age and process of disease [M( P25 ~ P75) ] were 4( 2 ~ 6) y,4( 2 ~ 12) m,separately.Sex ratio was 0. 4∶ 1( 12 boys and 23 girls). 38 cases( 84. 44%) of them were suffered from perianal,followed by genital 17. 78%. 8 cases( 17. 78%) had multiple sites involed,the rest( 82. 22%) are single site,especially anal( 66. 67%). There was no statistical difference between genders on lesion distribution. HPV positive test for 24 cases,the positive rate was 53. 33%. The most common type was HPV6,fol-lowed by HPV11,HPV7,and HPV59 + HPV6. The virus type has no statistically significant differences between genders except HPV6. Conclusion HPV6 or 11 were the main genotype to cause Child ano-genital warts,additionally,the ratio of cutaneous type infection may be underestimated.
