从处方看易致肝损伤中药饮片临床使用的合理性

摘　　要：目的:了解易引起肝损伤中药的处方现状,并分析其使用的合理性。方法:通过文献检索,选取了文献报道多、有明确肝损伤的中药13种,从我院2014年第3季度中药处方中随机抽取3000张处方,采集含肝损伤中药的处方信息,运用计量法和统计学方法对所采集的处方信息进行分析。结果:含有13种易引起肝损伤中药处方数占抽查处方总数的58.0%。频率较高的前5种为柴胡、栀子、苍术、何首乌、川楝子。13种中药都出现了不同程度的超剂量现象,其中何首乌超量最为严重,占80.6%;应用于肝脏疾病的处方有205种,占11.8%;肝损伤中药联用处方537张,占30.9%。结论:易引起肝损伤中药在临床使用广泛,存在肝损伤中药超剂量使用、治疗肝脏疾病及联合用药现象。Objective:To understand the present situation of liver injury induced by prescription of traditional Chinese medicine,and analyze the rationality of its use.Methods:Through the literature search,we selected 13 kinds of traditional Chinese medicines which have clear liver injury,3000 prescriptions were randomly selected from our hospital in the third quarter of 2014,collected and analyzed the prescription information.Results:The number of prescriptions containing 13 kinds of Chinese medicines that cause liver damage accounted for 58.0% of the total number of prescriptions.The top 5 frequencies are Chaihu(radix bupleuri),Zhizi(fructus gardeniae),Cangzhu(rhizoma atractylodis),Heshouwu(polygonum multiflorum),Chuanlianzi(szechwan Chinaberry fruit).There were different degrees of overdose in 13 kinds of traditional Chinese medicines,among which Heshouwu(polygonum multiflorum) was the most serious,accounting for 80.6%;A total of 205 kinds of prescriptions for liver diseases,accounting for 11.8%;A total of 537 cases of liver injury combined with prescriptions,accounting for 30.9%.Conclusion:Traditional Chinese medicine induced liver injury were used widely in clinical.There were phenomenon of super dose,treatment of liver diseases,and drug combination in using traditional Chinese medicine induced liver injury.
