养阴益气合剂治疗结缔组织病继发间质性肺病60例临床观察

摘　　要：目的:探讨养阴益气合剂治疗结缔组织病继发间质性肺病的效果。方法:将60例患者随机分为对照组和观察组,对照组29例予以口服醋酸泼尼松片基础治疗,观察组31例在对照组治疗基础上增加口服养阴益气合剂的治疗,于治疗前、治疗12周后分别记录6分钟步行试验(6MWT)、肺功能等结果,且停药12周后再次随访6MWT,并进行疗效比较。结果:两组患者在治疗12周后肺功能(FVC、DLCO)较治疗前显著改善(P<0.05),观察组改善程度更为显著(P<0.01);治疗12周后,两组患者的6MWT较治疗前均有提高(P<0.05),观察组改善程度更为显著(P<0.01),且停药12周后观察组的6MWT仍优于对照组(P<0.05)。治疗后观察组中医证候评分显著改善(P<0.05),且优于对照组(P<0.05)。结论:养阴益气合剂治疗结缔组织病继发间质性肺病气阴两虚证患者有显著疗效,可明显缓解临床症状、改善肺功能、增强活动耐力、提高生活质量,值得进一步推广应用。Objective:To investigate the effects of Yangyin Yiqi Mixture in the treatment of connective tissue disease associated with interstitial lung disease (CTD-ILD).Methods:Sixty patients were randomly divided into the control group and the observation group.Twenty-nine cases in the control group were given the basic treatment of oral Prednisone Acetate Tablets while 31 cases in the observation group were treated with oral Yangyin Yiqi Mixture combined with the basic treatment of the control group.The results of the six-minute walk test (6MWT)and lung function were recorded before treatment and 12 weeks after treatment.After 12 weeks'withdrawal,6MWT was followed up again,and the curative effect was compared. Results:After 12 weeks of treatment,the pulmonary function (FVC,DLCO)of the two groups were significantly improved compared with before treatment (P<0.05),the observation group improved more significantly (P<0.01).After 12 weeks of treatment,the 6MWT of the two groups were increased compared with that before treatment (P<0.05),and the improvement in the observation group was more significant (P<0.01).After 12 weeks of withdrawal,the 6MWT of the observation group was still better than that of the control group (P<0.05).After treatment,the observation group's TCM syndromes score improved significantly (P<0.05).Conclusion:Yangyin Yiqi Mixture has remarkable curative effect in treating patients with connective tissue disease secondary to interstitial lung disease with deficiency of both Qi and yin.It can obviously relieve clinical symptoms, improve lung function,enhance activity endurance and improve the quality of life.It is worthy of further popularization and application.
