慢性乙型肝炎肝组织中CD4^+、CD8^+T淋巴细胞的表达与肝窦及窦间隙病理病变的相关性研究

摘　　要：目的探讨慢性乙型病毒性肝炎(CHB)肝组织中CD4^+、CD8^+T淋巴细胞的表达与肝窦病变的关系。方法随机选择2013年7月至2016年12月蚌埠医学院第一附属医院慢性乙型病毒性肝炎(CHB)60例肝脏组织标本,另选择10例正常人肝组织标本,全部肝组织标本皆采取苏木素-伊红(HE)及免疫组化CD4^+、CD8^+染色。光镜观察CD4^+、CD8^+免疫组化染色阳性结果与肝窦及窦周间隙病变的关系。结果肝窦及窦周间隙病变可表现为肝窦阻塞、狭窄、毛细血管化等。CD4^+、CD8^+阳性表达与肝窦及窦周间隙病变积分密切相关。肝窦病变积分0-1分,少数CHB患者CD4^+、CD8^+阳性;肝窦病变积分1-2分,多数肝组织CD4^+、CD8^+淋巴细胞表达阳性;肝窦病理病变积分2-3分,其肝组织CD4^+、CD8^+淋巴细胞表达阳性或强阳性。CD4^+、CD8^+细胞表达加强,肝内血管病变随之加重,CD4^+与CD8^+阳性表达率与肝窦病变的积分组间相关性比较均有统计学差异(P<0.01)。结论肝组织内CD4^+、CD8^+阳性表达与肝窦及窦周间隙病变积分呈正相关。Objective to explore the relationship between the expression of CD4^+ and CD8^+T lymphocytes in liver tissue of chronic hepatitis B(CHB) and sinus lesions. Methods 60 cases of chronic hepatitis B(CHB) in the first affiliated Hospital of Bengbu Medical College were randomLy selected from July 2013 to December 2016, and 10 cases of normal liver tissue specimens were selected. All liver specimens were stained with hematoxylin-eosin(HE) and immunohistochemical CD4^+ and CD8^+.The correlation between positive results of immunohistochemical staining of CD4^+ and CD8^+ and liver sinus and peritoneal space lesions was observed by light microscopy. Results The pathological changes of hepatic sinus and sinus space can be manifested as hepatic sinus obstruction, stenosis and capillary. Positive expressions of CD4^+ and CD8^+ were closely related to the integral of liver sinus and peritoneal space lesions. Hepatic sinus lesions were scored 0-1, and CD4^+ and CD8^+ were positive in a few CHB patients. Liver sinus lesions were divided into 1-2 points, and most of the liver tissues were positive in CD4^+ and CD8^+ lymphocytes. Hepatic sinus pathological changes were scored by 2-3 points, and the liver tissues of CD4^+ and CD8^+ lymphocytes were positive or strongly positive. Increased expression of CD4^+ and CD8^+ cells led to increased intrahepatic vascular lesions, and the correlation between positive expression rates of CD4^+ and CD8^+ and hepatic sinus lesions was statistically different(P<0.01). Conclusion positive expression of CD4^+ and CD8^+ in liver tissue is positively correlated with the integral of hepatic sinus and peritoneal space lesions.
