摘 要:目的分析针刀镜微创术联合独活防萆冲剂对膝骨性关节炎患者的膝关节功能、疼痛、血沉、C-反应蛋白影响。方法取我院2017年01月至2018年3月收治的膝骨性关节炎患者120例分成3组,每组40例。中药组:常规内服中药独活防萆冲剂治疗;针刀镜组:单纯采用针刀镜治疗;联合组:采用针刀镜微创术治疗,并从术后第2天开始中药独活防萆冲剂口服。比较三组患者膝关节功能、疼痛、血沉、C-反应蛋白水平。结果治疗前三组患者膝关节功能、疼痛评分差异比较无统计学意义(P>0.05)。术后联合组患者膝关节功能评分高于中药组、针刀镜组(P<0.05),疼痛评分小于中药组、针刀镜组(P<0.05)。治疗前三组患者血沉、C-反应蛋白水平差异比较无统计学意义(P>0.05)。治疗后联合组患者ESR、C-PR显著低于中药组、针刀镜组(P<0.05)。结论针刀镜微创术联合独活防萆冲剂治疗膝骨性关节炎能显著降低术后疼痛,保护关节功能,改善血沉,减少炎症。Objective to analyze the effect of minimally invasive needle knife microscopy combined with Bixie granule on knee joint function, pain, blood sedimentation and C-reactive protein in patients with knee osteoarthritis. Methods 120 patients with knee osteoarthritis treated in our hospital from January 2017 to March 2018 were divided into 3 groups, with 40 cases in group. Traditional Chinese medicine group had conventional oral Chinese medicine alone live anti-Bixie granule treatment. Needle knife mirror group had simple needle knife mirror treatment. Combined group had minimally invasive surgery with needle-knife microscopy and began to oral Chinese medicine alone antibixie granuleas the 2 nd day after the operations. The levels of knee function, pain, blood sedimentation and c-reactive protein were compared between the three groups. Results there was no significant difference in knee function and pain scores among the three groups before treatment(P>0.05). The knee function score of the combined group was higher than that of the traditional Chinese medicine group and the acupotomicroscopy group(P<0.05), while the pain score was lower than that of the traditional Chinese medicine group and the acupotomicroscopy group(P<0.05). Before treatment, the differences in blood sedimentation and c-reactive protein levels in the three groups were not statistically significant(P> 0.05). After treatment, ESR and c-pr of the combined group were significantly lower than those of the traditional Chinese medicine group and the acupotomology group(P< 0.05). Conclusion the treatment of knee osteoarthritis by minimally invasive needle knife microscopy combined with Bixie granule can significantly reduce postoperative pain, protect joint function, improve erythrocyte sedimentation rate and reduce inflammation.
