导乐分娩配合自由体位助产对初产妇产程及自然分娩结局的作用

摘　　要：目的研讨初产妇接受导乐分娩与自由体位助产对产程进展、自然分娩结局的影响。方法选择2017年8月至2018年8月在我院产科分娩的680例初产妇参与实验观察,同时随机分配初产妇入组,Ⅰ组(n=340)接受传统卧位待产,Ⅱ组(n=340)接受导乐分娩配合自由体位助产,分析评估两组的产程进展情况以及自然分娩结局。结果Ⅰ组与Ⅱ组产妇在第三产程时间上的比较,无统计学意义(P> 0.05),但对比Ⅰ组,Ⅱ组产妇在第一产程、第二产程及总产程时间上均显著缩短,比较有统计学意义(P<0.05)。Ⅱ组产妇的自然分娩率较Ⅰ组产妇显著提高,产后出血量较Ⅰ组产妇显著减少,新生儿窒息率、剖宫产率较Ⅰ组产妇显著下降,比较有统计学意义(P<0.05)。结论对初产妇实施导乐分娩配合自由体位助产,能够促进产程进展,改善母婴结局,值得推荐。Objective To investigate the effects of primiparas on guided labor delivery and free positional midwifery on labor progression and natural delivery outcomes. Methods A total of 680 primiparas from the Department of Obstetrics and Gynecology in our hospital from August 2017 to August 2018 were enrolled in the study. The primipara women were randomly assigned to participate in the study. Group Ⅰ(n=340) received traditional gestation, and group Ⅱ(n=340) Received guided delivery and free position delivery, analysis and evaluation of the progress of the two groups and the outcome of natural delivery. Results There was no significant difference in the third stage of labor between the group Ⅰ and group Ⅱ(P>0.05), but compared with the group Ⅰ, the group Ⅱ had significant time in the first stage of labor, the second stage of labor and the total labor time. Shorted, it was statistically significant(P<0.05). The natural delivery rate of the women in group Ⅱ was significantly higher than that in group Ⅰ. The postpartum hemorrhage was significantly lower than that in group Ⅰ. The neonatal asphyxia rate and cesarean section rate were significantly lower than those in group Ⅰ(P<0.05). Conclusion The implementation of guided delivery and free body delivery for primipara can promote the progress of labor and improve maternal and infant outcomes. It is recommended.
