B型超声引导下臂丛神经阻滞麻醉的临床应用效果分析

摘　　要：目的探讨B型超声引导下臂丛神经阻滞麻醉的临床应用效果。方法选取2017年3月至2018年3月在温州医学院附属苍南医院骨科行上肢手术治疗的患者84例,采用随机数字表法分为对照组和研究组,每组42例。对照组使用常规盲探式操作方法进行臂丛神经阻滞麻醉,研究组使用B型超声引导下臂丛神经阻滞麻醉,比较两组患者麻醉有效率、神经阻滞相关指标及并发症发生情况。结果研究组麻醉有效率为97.6%,明显高于对照组的80.9%,差异有显著性(P<0.05);研究组的阻滞起效时间、局麻药使用剂量均明显低于对照组,而镇痛连续时间明显长于对照组,差异均有显著性(P<0.05)。研究组臂丛神经阻滞麻醉相关并发症发生率明显低于对照组(2.3%vs 19.0%),差异有显著性(P<0.05)。结论 B型超声引导下臂丛神经阻滞麻醉的效果优于常规盲探操作,且相关并发症发生率较低,值得临床推广应用。Objective To explore the clinical effect of brachial plexus block anesthesia guided by B-mode ultrasound. Method 84 cases ofupper limb surgery in our hospital department of orthopedics, March 2017 -2018 years, were randomly divided into two groups accordingto the digital table method. The two groups were all anesthetized, the control group used the conventional blind exploration operationmethod, the study group used B ultrasound guided brachial plexus block anesthesia, compared two groups of anesthesia efficiency, nerveblock related indicators and incidence of complications. Result The effective rate of anaesthesia in the study group was 97.6%, whichwas significantly higher than the control group’s 80.9%. In the study group, the starting time of block and the dosage of local anestheticwere significantly lower than that of control group, while the analgesic duration was significantly longer than that of control group, and thedifference was significant (P<0.05). The incidence of anaesthesia related complications of brachial plexus block in the study group wassignificantly lower than that of the control group (2.3 % vs 19.0 %), and the difference was significant (P<0.05). Conclusion The operationpatients choose the brachial plexus block anesthesia guided by B-ultrasound, the effect of the operation anesthesia is more effective than theconventional blind method. The operation doctor’s puncture vision is clearer and the accuracy rate is high, and the related index of nerveblock is better. It should be widely spread and applied in the clinic.
