降钙素原在Ⅰ型主动脉夹层术后肺部感染早期诊断中的价值

摘　　要：目的探讨Ⅰ型主动脉夹层术后发生肺部感染的患者围术期降钙素原(PCT)的变化及临床应用价值。方法回顾性分析我院2015年9月至2017年11月的Ⅰ型主动脉夹层(Debakey分型)手术后早期出现肺部感染的患者(感染组)23例为研究对象,选取同期未出现肺部感染的Ⅰ型主动脉夹层手术后患者(非感染组)28例为对照,分别于手术前、术后即刻、术后24 h、72 h分别检测血PCT水平,进行比较分析。结果 2组在术后即刻、术后24 h及术后72 h的血PCT水平均高于手术前,差异有统计学意义(P <0. 05);感染组术后24 h、72 h血PCT水平明显高于非感染组,差异有统计学意义(P <0. 05)。结论Ⅰ型主动脉夹层术后发生早期肺部感染时PCT水平增高,作为判断感染的重要指标,可更早期鉴别出高危患者,及时诊治。Objective To investigate the change and clinical value of procalcitonin in postoperative pulmonary infection in type Ⅰ aortic dissection(Debakey types)patients.Methods Totally 23 patients (infection group),who complicated with postoperative pulmonary infection in type Ⅰ aortic dissection from September 2015to November 2017,were retrospectively analyzed.28cases without postoperative pulmonary infection of the same period as control group.The procalcitonin level was detected before the surgery,immediately after operation,24h and 72h after surgery.Then to analysis the data.Results In two groups,the level of procalcitonin was obviously higer at immediately after operation,24h and 72h after surgery than that before operation(P <0.05);the level of procalcitonin was significantly higher in 24h and 72h after surgery in infection group than that in no infection group(P <0.05).Conclusion The level of procalcitonin increases in the early period after type I aortic dissection operation.As an important index of infection,procalcitonin can early distinguishing the high -risk patients and give treatment timely.
