替罗非班治疗急性非ST抬高心肌梗死患者临床研究

摘　　要：目的探讨急性非ST抬高心肌梗死患者(非冠脉血运重建)应用国产替罗非班的疗效。方法选择商丘市第一人民医院2015年1月—2015年12月收治的急性非ST抬高的心肌梗死(NSTEMI)患者100例(非冠脉血运重建),随机分为两组,观察组在口服拜阿司匹林、氯吡格雷基础上加用国产替罗非班,对照组口服拜阿司匹林、氯吡格雷,对比观察两组心肌坏死标记物、心电图ST段回落情况,血小板数减少、出血情况及心血管恶性事件(恶性心律失常、再梗死及出血)的发生率。结果观察组心电图ST段回落程度为(0. 10±0. 02)mm,显著优于对照组[(0. 16±0. 05)mm],心肌坏死标记物达峰时间明显提前,两组住院期间血小板减少出血情况、凝血功能差异无统计学意义(P> 0. 05),住院期间及随访1个月心血管恶性事件如恶性心律失常及再梗死等的发生率,观察组明显低于对照组,差异有统计学意义(P <0. 05)。结论对非冠脉血运重建急性非ST抬高心肌梗死患者,在口服拜阿司匹林、氯吡格雷基础上加用国产替罗非班,能强化标准治疗的抗血小板、抗凝效果,心血管恶性事件的发生率明显降低,而出血风险、血小板减少并没有增加。疗效安全确切,可在临床推广应用。Objective To explore the efficacy of domestic tirofiban in patients with acute non -st elevation myocardial infarction (non -coronary blood circulation reconstruction).Methods Our hospital acute non ST elevation myocardial infarction (NSTEMI)patients,100cases of (non coronary reascularization),randomly divided into two groups,the observation group based on worship of oral aspirin and clopidogrel for spot,had used domestic control group oral aspirin and clopidogrel,compared two groups of myocardial necrosis markers,ecg ST segment fell back,platelet count reduction,bleeding and cardiovascular malignant events (malignant arrhythmia,infarctionand hemorrhage)incidence. Results Observation group degree of ecg ST segment fell significantly better than that of the control group,the myocardial necrosis markers tmax significantly ahead of time,two groups of hospitalization thrombocytopenia bleeding,blood coagulation function no statistically significant difference (P <0.05),hospitalization and follow -up 1 month cardiovascular malignant events such as the incidence of malignant arrhythmia and infarction again in observation group significantly lower than those the control group,the difference was statistically significant (P <0.05).Conclusion For non -coronary reascularization acute ST elevatio.n myocardial infarction patients,based on worship of oral aspirin and clopidogrel combined with domestic for spot,can strengthen the antiplatelet and anticoagulant effect of standard treatment,significantly reduce the incidence of cardiovascular malignant events,but did not increase bleeding risk,thrombocytopenia.The curative effect is safe and accurate,and can be applied in clinic.
