摘 要:目的探究FoxP3^+Treg细胞和炎症因子在宫颈病变中的表达情况及与宫颈病变的相关性。方法选取23例就诊于同济大学附属第十人民医院的宫颈病变患者作为病例组,另选取同期来我院体检的23例健康女性作为对照组,并根据HPV感染的分型分为高危组和低危组。对受试人员进行妇科检査并取宫颈上皮细胞,分析人乳头瘤病毒(humanpapillomavims,HPV)感染情况。抽取患者静脉血并检测Foxp^3Treg细胞的表达情况,检査受试人员宫颈灌洗液中炎症因子的表达情况并分析其与宫颈病变的相关性,同时应用免疫组化分析对患者切除的宫颈组织进行Foxp 3蛋白的检测。结果病例组人群的白细胞介素(interleukin,IL)-12、干扰素(interferon,IFN)7水平明显更低(P<0.05),而IL-10、IL>4和转化生因子(transforming growth factor,TGF)-β1的水平较正常受试人员显著更高(P<0.05);髙危组患者中Foxp^3+ Treg阳性的比例髙于低危组V=31.360,P<0.05)。不同HPV感染分型的比较表明,低危组患者的IL-10、IL-4、TGF-β1和外周血Foxp^3+Treg细胞百分比均显著低于髙危组患者(P<0.05),而IL-12、IFN-γ的水平则显著高于髙危组(P<0.05)。结论Foxp^3+Treg和炎症因子的水平与宫颈病变的严重程度有明显关系,宫颈病变时二者表达增多,提示其可针对性地指导宫颈病变的诊断与治疗。Objective To investigate FoxP3^+ Treg cells and inflammatory factors in cervical lesions. Methods All of 23 patients with cervical lesions in our hospital were chosen as the case group, while another 23 patients were chosen at the same time as control group. According to their HPV infection, patients were divided into high risk group and low risk group. Gynecological examinations were performed on the subjects and cervical epithelial cells were taken to analyze HPV infection. The patient's venous blood was extracted and FoxP3^+ Treg cells were detected. Inflammatory factors in the cervical lavage fluid of the subjects were examined and their correlation with the cervical lesions was analyzed. FoxP3^+ protein was detected by immunohistochemical a-nalysis. Results The levels of IL-12 and IFN-γ in the case group were significantly lower (P < 0. 05 ) , while the levels of IL-10, IL-4, and TGF-β1 were significantly higher than those of control group ( P < 0. 05 ) . The proportion of FoxP3^+ Treg positive in high-risk groups was significantly higher than that in the low risk group(X^2 = 31. 360,P <0.05) . The percentage of IL-10,IL-4,TGF-β1 ,and peripheral blood Foxp3 + Treg cells in the low risk group were significantly lower than that in the high risk group (P <0. 05) , while the levels of IL-12 and IFN-γ were significantly higher than that in the high risk group(P <0. 05). Conclusion The level of Foxp3 + Treg and inflammatory factors has a significant relationship with the severity of cervical lesions, which may guide the diagnosis and treatment of cervical lesions.
