烧伤创疡再生医疗技术联合自体点柱状微粒皮种植术治疗老年烧伤的临床研究

摘　　要：目的探讨烧伤创疡再生医疗技术联合自体点柱状微粒皮种植术治疗老年烧伤的临床疗效,总结治疗体会。方法对2016年1月—2018年1月延安大学附属医院烧伤整形手外科收治的80例老年烧伤患者采用烧伤创疡再生医疗技术联合自体点柱状微粒皮种植术治疗,观察治疗效果。结果 80例患者中除3例患者放弃治疗外,其余患者创面均完全愈合,其中浅Ⅱ度烧伤创面愈合时间为7～14 d、深Ⅱ度烧伤创面愈合时间为15～22 d、Ⅲ度烧伤创面愈合时间为30～35 d,且治疗过程中均无感染等并发症发生。创面愈合后,除3例Ⅲ度烧伤患者愈后皮肤留有增生性瘢痕,但未影响肢体功能外,其余患者愈后皮肤均无明显瘢痕增生,皮肤弹性较好。结论烧伤创疡再生医疗技术联合自体点柱状微粒皮种植术治疗老年烧伤,可有效促进创面愈合,降低瘢痕增生率、肢体残疾率及死亡率,疗效显著,值得临床推广应用。Objective To explore the efficacy of MEBT/MEBO combined with autologous implantation of cylinder-like micro-skin in treating elderly burn patients,and summarize the treatment experience.Methods Eighty elderly burn patients,admitted into the Department of Burns,Plastics and Hand Surgery,Yan'an University Affiliated Hospital from January 2016 to January 2018,were treated with MEBT/MEBO in combination with autologous implantation of cylinder-like micro-skin.The therapeutic effect was observed.Results Except for 3 patients who gave up treatment,the wounds of all the other patients healed completely.The wound healing time for the second degree burns was 7 to 14 days, for deep second degree 15 to 22 days and for third degree 30 to 35 days.No infection or complication occurred during the treatment course.After wound healing,3 cases of Ⅲ degree wounds got scar hyperplasia,but the scar did not affect their limb functions.No obvious scar hyperplasia was observed in other patients and their recovered skin had good elasticity. Conclusion MEBT/MEBO combined with autologous implantation of cylinder-like micro-skin has good curative effect on burns in elderly patients.It can effectively promote wound healing,and reduce scar hypertrophy rate,limb disability rate and mortality,deserved to be promoted in clinical application.
