右归丸联合西药治疗老年高血压(?肾阳亏虚?)随机平行对照研究

摘　　要：[目的]观察右归丸联合西药治疗老年高血压(肾阳亏虚)疗效。[方法]使用随机平行对照方法,将150例门诊住院按就诊顺序号抽签方法随机分为两组。对照组75例氢氯噻嗪,50mg/次,1次/d;美托洛尔,150mg/次,1～2次/d,据血压单独或联合使用,调整剂量。治疗组75例右归丸(地黄熟40g,山药20g,山茱萸、枸杞子、肉桂各15g,菟丝子、鹿角胶、杜仲各20g,附子制10g),失眠多梦加酸枣仁、远志、麦冬各15g;头痛中加川芎15g,细辛8g;小便清长加茯苓、泽泻各15g;腰膝酸软加牛膝、五加皮各15g,水煎300mL,1剂/d,早晚分服;西药治疗同对照组。连续治疗7d为1疗程。观测临床表现、舒张压、收缩压、中医证候积分、不良反应。连续治疗4疗程(28d),判定疗效。[结果]治疗组显效40例,有效30例,无效5例,总有效率93.33%;对照组显效36例,有效24例,无效15例,总有效率80.00%;治疗组疗效优于对照组(P<0.05)。舒张压、收缩压、中医证候积分两组均有降低(P<0.01),治疗组降低多于对照组(P<0.01)。[结论]右归丸联合西药治疗老年高血压(肾阳亏虚),可提高降压效果,改善伴随症状,值得推广。[Objective]To observe the curative effect of Youguiwan combined with western medicine on senile hypertension (kidney yang deficiency).[Methods] 150 out-patient inpatients were randomly divided into two groups according to the order of visiting.75 cases in the control group were treated with hydrochlorothiazide 50 mg/time,once a day,metoprolol 150 mg/time,once to twice a day,and the dosage was adjusted according to the blood pressure.In the treatment group,75 cases were treated with Youguiwan (Shudihuang 40g,Shanyan 20g,Shanzhuyu,Gouqizi,Rougui each 15g,Tusizi,Lujiaojiao,Duzhong each 20g,Zhifuzi10g),insomnia,add Suanzaoren,Yuanzhi,Maidong each 15g;headache,add Chuanxiong 15g,Xixin 5g;frequency of urine,add Fulng,Zexie each 15g;soreness and weakness of waist and knees,add Niuxi,Wujiapi each 15g,decocted 300 mL,1 doses of/d,sooner or later divided;western medicine treatment with the control group.Continuous treatment of 7d for 1courses.Clinical manifestations,diastolic blood pressure,systolic blood pressure,TCM syndrome scores,and adverse reactions were observed.Continuous treatment of 4 courses (28d),determine the curative effect.[Results]The total effective rate was 93.33%in the treatment group of 40cases,30cases effective,5 cases ineffective;36 cases in the control group,24 cases effective,15 cases ineffective,the total effective rate was 80.00%;the curative effect of the treatment group was better than that of the control group (P<0.05).Diastolic blood pressure,systolic blood pressure and TCM syndrome score were decreased in both groups (P<0.01),and the decrease in treatment group was more than that in control group (P<0.01). [Conclusion]Yougui pill combined with western medicine in the treatment of senile hypertension (deficiency of kidney yang)can improve the antihypertensive effect,improve the accompanying symptoms,and is worth promoting.
