补肺涤痰汤+参芪十一味颗粒联合沙美特罗替卡松治疗慢性阻塞性肺疾病稳定期(肺肾两虚)随机平行对照研究

摘　　要：[目的]观察补肺涤痰汤+参芪十一味颗粒联合沙美特罗替卡松治疗慢性阻塞性肺疾病稳定期(肺肾两虚)疗效。[方法]使用随机平行对照方法,将118例门诊按就诊顺序号方法随机分为两组。对照组59例沙美特罗替卡松气雾剂,1吸/次,2次/d,治疗后漱口。治疗组59例补肺涤痰汤(地龙20g,莱菔子30g,云茯苓、地黄熟各15g,地黄生、沙参生、杏仁、桃仁、白术炒、苏子、枇杷叶炙、甘草炙各10g),水煎300mL,1剂/d,早晚温服;参芪十一味颗粒,2g/次,2次/d,口服;沙美特罗替卡松治疗同对照组。连续治疗3个月为1疗程。观测临床表现、第1s用力呼吸容积(FEV1)、FEV1/用力肺活量(FVC)、健康调查简表(SF-36)、不良反应。治疗1疗程(3个月),判定疗效。[结果]FEV_1,FEV_1/FVC,SF-36两组均有改善(P<0.01),治疗组改善优于对照组(P<0.05,P<0.01)。不良反应发生率治疗组11.86%与对照组15.25%比较无显著差异(P>0.05)。[结论]补肺涤痰汤+参芪十一味颗粒联合沙美特罗替卡松治疗慢性阻塞性肺疾病稳定期(肺肾两虚),疗效满意,无严重不良反应,值得推广。[Objective] To observe the curative effect of Bufei Ditan decoction+Shenqi eleven flavor granules combined with salmeterol and roticasone on chronic obstructive pulmonary disease(COPD) in stable phase(Feishen Liangxu).[Methods] 118 outpatients were randomly divided into two groups according to the order of visiting.In the control group,59 cases of Salmeterol Xinafoate and Fluticasone Propionate Aerosol,1 times/times,2 times/d,gargle after treatment.Treatment group 59 cases of Bufei Ditan decoction(Dilong 20 g,Laifuzi 30 g,Yunfuling,Shudihuang each 15 g,Shengdihuang,Shengshashen,Xingren,Chaobaozhu,Suzi,Zhipipaye,Zhigancao each 10 g),decoction of 300 mL,one dose per day,warm in the morning and evening;Shenqi eleven granules,2 g/time,2 times a day,oral administration;Sameterol Roticasone treatment;Treated with the control group.Continuous treatment for 3 months for 1 courses.Clinical manifestations,FEV1,FVC,SF-36,adverse reactions were observed.1 courses of treatment(3 months) were used to determine the curative effect.[Results] FEV1,FEV1/FVC and SF-36 were improved in both groups(P<0.01).The improvement in treatment group was better than that in control group(P<0.05,P<0.01).There was no significant difference in the incidence of adverse reactions between the treatment group 11.86% and the control group 15.25%(P>0.05).[Conclusion] Bufei Ditan decoction + Shenqi eleven flavor granules combined with Salmeterol and Roticasone in the treatment of chronic obstructive pulmonary disease in stable phase(deficiency of both lung and kidney),the curative effect is satisfactory,no serious adverse reactions,it is worth promoting.
