辨证分型联合西药治疗小儿咳嗽变异性哮喘随机平行对照研究

摘　　要：[目的]观察辨证分型联合西药治疗小儿咳嗽变异性哮喘疗效。[方法]使用随机平行对照方法,将86例门诊患者按随机数字表法随机分为两组。对照组43例氨茶碱,4～6mg/kg,3次/d;酮替酚,0.5～1mg/次,2次/d;孟鲁司特钠,5mg/次,咀嚼,每晚顿服。治疗组43例辨证分型,肺热-定喘汤(杏仁、僵蚕、橘皮各6g,芦根各9g,玄参8g,葶苈子3g,黄芩10g,浙贝母12g);肺寒-苏陈九宝汤(苏子9g,陈皮、川朴各12g,杏仁、防风、苍耳子、麻黄、姜干各3g,桑皮、地肤子各8g,旋覆花、前胡各6g);肝火犯肺-泻白黛蛤散(芦根12g,黄芩、橘红、地骨皮、杏仁各3g,丹皮、沙参、麦冬各6g,桑皮9g,钩藤10g);肺气虚-补肺汤(黄芪、姜生、大枣、白芍、沙参、五味子、人参、白术、灵芝各9g,虫草、防风、桂枝各6g);西药治疗同对照组。连续治疗1个月为1疗程。观测临床表现、喘息消失时间、呼吸困难消失时间、咳嗽消失时间、咳痰消失时间、不良反应。治疗1疗程(1个月),判定疗效。[结果]治疗组痊愈30例,好转11例,无效2例,总有效率95.35%;对照组痊愈12例,好转21例,无效10例,总有效率76.74%;治疗组疗效优于对照组(P<0.05)。喘息消失时间、呼吸困难消失时间、咳嗽消失时间、咳痰消失时间治疗组短于对照组(P<0.01)。[结论]辨证分型联合西药治疗小儿咳嗽变异性哮喘,疗效满意,起效快,无严重不良反应,值得推广。[Objective] To observe the curative effect of syndrome differentiation combined with western medicine in treating children cough variant asthma.[Methods] 86 outpatients were randomly divided into two groups by random number table method.The control group consisted of 43 cases of aminophylline,4~6 mg/kg,3 times a day;ketotifen,0.5~1 mg/time,2 times a day;montelukast sodium,5 mg/time,chewing,and taking it every night.In the treatment group,43 cases were classified according to syndrome differentiation,including Feire-Dingchuan decoction(Xingren,Jiangcan,Jupi each 6 g,Lugen each 9 g,Xuanshen 8 g,Tinglizi 3,Huangqin 10 g,Zhebeimu 12 g);Feihan-Suchenjiubao decoction(Suzi 9 g,Chenpi,Chuanpo each 12 g,Xiingren,Fangfeng,Cangerzi,Mahuang,Ganjing each 3 g,Sangpi,Difuzi each 8 g,Xuanfuhua,Qianhu each 6 g) Liver Fire Invades Lung-Xiebai Daijie Powder(Lugen 12 g,Huangqin,Juhong,Digupi,Xingren each 3 g,Danpi,Shashen,Maidong each 6 g,Sangpi 9 g,Gouteng 10 g);Lungqi deficiency-Bufei decoction(Huangqi,Shengjiang,Dazzao,Baishao,Shashen,Wuweizi,Renshen,Baizhu,Lingzhi each 9 g,Chongcao,Fangfeng,Guizhi each 6 g);western medicine treatment with the control group.Continuous treatment for 1 months for 1 courses.The clinical manifestations,disappearance time of wheezing,disappearance time of dyspnea,disappearance time of cough,disappearance time of sputum and adverse reactions were observed.1 courses of treatment(1 months) were used to determine the curative effect.[Result] In the treatment group,30 cases were cured,11 cases were improved,2 cases were ineffective,and the total effective rate was 95.35%.In the control group,12 cases were cured,21 cases were improved and 10 cases were ineffective,the total effectiverate was 76.74%.The curative effect of the treatment group was better than that of the control group(P<0.05).The disappearance time of wheezing,dyspnea,cough and sputum in the treatment group was shorter than that in the control group(P<0.01).[Conclusion] Syndrome differentiation combined with western medicine in the treatment o
