补肾养血汤联合重组人促红素治疗肾性贫血(气血两虚)随机平行对照研究

摘　　要：[目的]观察补肾养血汤联合重组人促红素治疗肾性贫血(气血两虚)疗效。[方法]使用随机平行对照方法,将42例门诊及住院患者按随机数字表法随机分为两组,优质蛋白、低磷饮食,足够热量,维生素B族、叶酸、琥珀酸亚铁、活性钙等,控制感染、血压及纠正水、电解质酸碱失调。对照组20例重组人促红素6000IU/次,3次/周,皮下注射。治疗组22例补肾养血汤(黄芪30g,党参20g,当归12g,地黄熟30g,黄精、龟板各20g,山药30g,山茱萸10 g,白术炒15g,菟丝子、补骨脂、仙茅、仙灵脾各10g,丹参15g,陈皮8g,砂仁6g),水煎300mL,1剂/d,2次/d;重组人促红素治疗同对照组。连续治疗8周为1疗程。观测临床表现、血清肌酐(Scr)、尿素氮(BUN)、血红蛋白(Hb)、不良反应。治疗1疗程(8周),判定疗效。[结果]治疗组临床痊愈10例,显效6例,有效4例,无效1例,总有效率95.45%;对照组临床痊愈3例,显效5例,有效5例,无效7例,总有效率65.00%;治疗组疗效优于对照组(P<0.05)。Scr,BUN,Hb两组均有改善(P<0.05,P<0.01),治疗组改善优于对照组(P<0.05)。[结论]补肾养血汤联合重组人促红素治疗肾性贫血(气血两虚),疗效满意,无严重不良反应,值得推广。[Objective] To observe the therapeutic effect of Bushen Yangxue decoction combined with recombinant human erythropoietin on renal anemia(Qi and blood two deficiency).[Method] Using randomized parallel contrast methods,42 outpatients and inpatients were randomly divided into two groups according to random number table,high-quality protein,low-phosphorus diet,sufficient calories,vitamin B group,folic acid,ferrous succinate,active calcium,etc,to control infection,blood pressure and correct water,electrolyte acid and alkali disorders.The control group consisted of 20 cases of recombinant human erythropoietin 6000 IU/,3 times/week,subcutaneous injection,22 cases of kidney nourishing and blood nourishing decoction in treatment group(Hauqngi 30 g,Dangshen 20 g,Danggui 12 g,Shudihuang 30 g,Huangjing,Guiban each 20 g,Shanyao 30 g,Shanzhuyu 10 g,Chaobaizhu 15 g,Tusizi,Buguzhi,Xianmao,Xianlingpi each 10 g,Danshen 15 g,Chenpi 8 g,Sharen 6 g),water fried 300 mL,1 doses/d,2 times/d;recombinant human erythropoietin treatment in the same control group.Continuous treatment for 8 weeks was 1 courses.Observation of clinical manifestations,serum creatinine(Scr),urea nitrogen(BUN),hemoglobin(Hb) and adverse reactions.1 courses of treatment(8 weeks),judge curative effect.[Results] 10 cases were cured in the treatment group,6 cases were markedly effective,4 cases were effective,1 cases were ineffective,the total effective rate was 95.45%;In the control group,3 cases were cured,5 cases were markedly effective,5 cases were effective,7 cases were ineffective,and the total effective rate was 65%;The curative effect of the treatment group was better than that of the control group(P<0.05).Scr,BUN and Hb were improved in two groups(P<0.05,P<0.01),the treatment group improved better than the control group(P<0.05).[Conclusion] Tonifying kidney and nourishing blood decoction combined recombinant human erythropoietin in the treatment of renal anemia(Qi and blood two deficiency),the curative effect is satisfactory without serious adverse reactions
