针刺治疗颈源性肩周炎随机平行对照研究

摘　　要：[目的]观察针刺治疗颈源性肩周炎疗效。[方法]使用随机平行对照方法,将120例住院及门诊患者按就诊顺序号简单随机分两组。对照组60例疼痛部位注射:阿司匹林、尼美舒利及舒林酸+2%盐酸+8mL丁卡因+3mL泼尼松龙+1mL维生素B1,1次/周。治疗组60例针刺(百会、合谷、神庭、外关、印堂、曲池、肩髃、阿是穴及内关等),结合病情配穴;首选1～5个穴位,0.5寸30号钢针针刺,每隔10min刺入一次,根据病情选择刺入次数,针刺时患者适当活动肩关节,产生紧张感。连续治疗2周为1疗程。观测临床表现、疼痛评分、肩关节功能评分、生活质量改善、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组显效41例,有效17例,无效2例,总有效率96.50%;对照组显效29例,有效22例,无效9例,总有效率85.00%;治疗组疗效优于对照组(P<0.05)。疼痛评分、肩关节功能评分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。生活质量改善两组均有改善(P<0.05,P<0.01),治疗组改善优于对照组(P<0.05,P<0.01)。[结论]针刺治疗颈源性肩周炎,疗效满意,无严重不良反应,值得推广。[Objective] To observe the therapeutic effect of acupuncture on treating scapulohumeral periarthritis.[Method] 120 inpatients and outpatients were randomly divided into two groups according to the order of visiting.In the control group,60 patients were injected with aspirin,nimesulide and sulindac+2% hydrochloric acid+8 mL tetracaine+3 mL prednisolone+1 mL vitamin B1 once a week.In the treatment group,60 cases of acupuncture(Baihui,Hegu,Shenting,Waiguan,Yintang,Quchi,Shoulder Condyle,Ashi and Neiguan) were combined with the condition of the disease.The first choice was 1-5 points,0.5 inch 30 steel needle,once every 10 minutes,according to the condition of the disease,the number of penetrations was selected,and the patients moved the shoulder joints appropriately to produce tension.Continuous treatment for 2 weeks was 1 courses.Clinical performance,pain score,shoulder function score,quality of life improvement and adverse reactions were observed.Continuous treatment of 2 courses,determine the curative effect.[Result] 41 cases were markedly effective in the treatment group,17 cases were effective,2 cases were ineffective,the total effective rate was 96.50%.29 cases were markedly effective in the control group,22 cases were effective,9 cases were ineffective,the total effective rate was 85.00%.The curative effect of the treatment group was better than that of the control group(P<0.05).Pain score and shoulder function score were improved in both groups(P<0.01),and the improvement in treatment group was better than that in control group(P<0.01).The quality of life improved in both groups(P<0.05,P<0.01),and the improvement in treatment group was better than that in control group(P<0.05,P<0.01).[Conclusion] Acupuncture has a satisfactory effect on treating scapulohumeral periarthritis of shoulder,and there is no serious adverse reaction.It is worth promoting.
