摘　　要：目的对比腹腔镜阑尾切除术(LA)与开腹阑尾切除术(OA)的临床治疗效果。方法回顾性总结2017年1月-2017年12月收治的221例诊断为急性阑尾炎,并行阑尾切除术的病例资料,所有病人根据个人的意愿分为OA组(n=83)和LA组(n=138),对比两组病人的术中出血量、手术时间、切口长度,以及术后切口疼痛程度、下床活动时间、肠功能恢复时间、腹腔残余感染率、伤口感染率、住院时间。结果LA组的平均术中出血量、手术时间、切口长度、疼痛级别、下床活动时间、肠功能恢复时间、住院时间、伤口感染率均明显低于OA组,差异具有统计学意义(P<0.05)。结论与OA相比,LA能够有效减少手术创伤,减轻术后疼痛程度,促进尽早恢复,减少切口感染率,缩短住院时间,值得在临床治疗中推广应用。Objective To compare the clinical effect of laparoscopic appendectomy (LA)to that of open appendectomy (OA).Methods We retrospectively summarized the clinical data of 221 patients with acute appendicitis in our hospital from January 2017 to December 2017.All patients were divided into two groups according to individual wish,83 cases who underwent LA were included in LA group,and 138 cases who underwent OA were included in OA group.Blood loss,operation time,length of incision,degree of postoperative pain,postoperative ambulation time,recovery time of bowel function,residual abdominal infection rate,incision infection rate and hospital stay were compared between two groups.Results The blood loss,operation time, length of incision,postoperative pain degree,incision infection rate,postoperative ambulation time,recovery time of bowel function,and hospital stay in LA group were significantly lower than those in OA group (P <0.05). Conclusion Compared with OA,LA can effectively reduce surgical injury,relieve postoperative pain,promote recovery,reduce incision infection rate,and shorten hospital stay.LA is worth to recommend in the clinical treatment of patients with appendicitis.
