摘　　要：目的探讨经食管实时三维超声心动图(RT-3D-TEE)在评价非瓣膜性房颤(NVAF)患者中-重度二尖瓣反流(MR)中的应用价值。方法 60例NVAF患者(轻度MR组40例,中-重度MR组20例)及健康对照组25例均行经胸二维超声心动图(2DTTE)和RT-3D-TEE检查,测量MR、左房内径(LAD)及二尖瓣参数。结果中-重度MR组与健康对照组比较,瓣环前外侧-后内侧径(DAlPm)、瓣环周长(C3D)、LAD增大、瓣环高度(H)、对合指数减小,差异有统计学意义(P<0.05)。NVAF患者出现中-重度MR时,对合指数、LAD所对应的ROC曲线下面积分别为(0.876±0.050)、(0.856±0.066)。结论对合指数、LAD诊断NVAF患者发生中-重度MR具有较高的特异度和灵敏度。Objective To explore the application value of real-time three-dimensional transoesophageal echocardiography(RT-3 D-TEE)quantitative analysis of moderate or severe mitral regurgitation(MR)in non-valvular atrial fibrillation(NVAF).Methods 60 patients were divided into no or mild MR group(n=40)and moderate or severe MR group(n=20),and another 25 patients without atrial fibrillation and MR were served as control group.All of the patients were examined through two-dimensional transthoracic echocardiography(2D-TTE)and RT-3D-TEE.MR,Left atrial diameter(LAD)and mitral valve parameters were analyzed.Results The anterolateral to posteromedial diameter(DAlPm),three dimensional circumference(C3D)and LAD were larger and the height(H),mitral valve coaptation index was smaller in the moderate or severe MR group than in the control group(P<0.05).When moderate or severe MR occurred in NVAF,the area under ROC curve of mitral valve coaptation index and LAD were respectively(0.876±0.050),(0.856±0.066).Conclusions The mitral valve coaptation index and LAD has high specificity and sensitivity in diagnosis of moderate or severe MR in NVAF.
