血液透析滤过联合罗匹尼罗对尿毒症患者不宁腿综合症的临床疗效观察

摘　　要：目的探讨血液透析滤过联合罗匹尼罗对尿毒症患者不宁腿综合征的疗效。方法将50例行血液透析的不宁腿综合征患者采用简单随机方法分成对照组和观察组。对照组予以常规血液透析治疗,每周3次,每次4 h,联合罗匹尼罗(0.25 mg/d)口服,观察组予以血液透析滤过治疗,每周3次,每次4 h,联合罗匹尼罗(0.25 mg/d)口服,疗程为12周。观察治疗前后患者精神卫生相关指标(包括不宁腿严重程度评分,抑郁、睡眠质量及生活质量评分)和临床检验指标(血清中全段甲状旁腺素、β_2-微球蛋白、血肌酐、血红蛋白含量)。结果治疗12周后,对照组和观察组患者不宁腿严重程度评分、抑郁评分、睡眠质量评分都较治疗前明显降低(P <0.05),观察组较对照组降低更为明显(P <0.05)。透析后对照组和观察组生活质量评分都明显增加,观察组较对照组增加更为明显(P <0.05)。透析后血清中全段甲状旁腺素及β2-微球蛋白含量观察组较对照组明显降低(P <0.05)。透析前后2组血肌酐和血红蛋白水平无明显差别(P> 0.05)。结论血液透析滤过联合罗匹尼罗对血液透析患者不宁腿综合征疗效明显。Objective To investigate the effectiveness of ropinirole combined with hemodiafiltration for restless legs syndrome in uremic patients. Methods Fifty patients with restless leg syndrome were divided into control group and experimental group by simple random method. The control group was given routine hemodialysis combined with ropinirole( 0.25 mg/day). The experimental group was treated with hemodialysis filtration combined with ropinirole( 0.25 mg/day) for 12 weeks. The changes of restlessness,depression,sleep quality and quality of life scores and the changes of total parathyroid hormone and β2-microglobulin in serum were observed before and after treatment. Results After 12 weeks of treatment,the severity score of the restless leg,depression score,and sleep quality score. in the control group and the experimental group was significantly lower than those before treatment( P < 0.05),and those in the experimental group decreased more significantly than in the control group( P < 0.05). The quality of life scores in the control group and the experimental group increased significantly,and after treatment,and those in the experimental group were significantly higher than in the control group( P < 0.05). After dialysis,the total parathyroid hormone and beta 2-microglobulin content in the experimental group were significantly lower than those in the control group( P < 0.05). There was no significant difference in serum creatinine and hemoglobin levels before and after dialysis in two groups( P > 0.05). Conclusions The combination ofropinirole and hemodiafiltration has obvious effect on restless legs syndrome in hemodialysis patients.
