摘 要:目的:探讨补肾活血方对骨关节炎小鼠局部淋巴管结构与分布的调节作用,并进一步探究其对关节退变的治疗效果。方法:清洁级雄性C57BL/6小鼠30只,随机分为3组,每组10只,假手术组仅打开小鼠右膝内侧皮肤及关节囊而不损伤其他结构;其余小鼠则切断内侧副韧带及内侧半月板前部致骨关节炎模型;补肾活血方组于造模后第2天开始以补肾活血方浓缩煎剂按0.2 mL/只的剂量灌胃;假手术组及模型组给予同等剂量0.9%氯化钠溶液干预,每天1次,持续12周。分别于4周及12周处死小鼠取右膝关节, Safranin-O染色观察关节组织的形态和结构;双免疫荧光组织化学染色法及全切片扫描系统检测小鼠局部淋巴管结构与分布情况。结果:术后12周,模型组小鼠膝关节呈现软骨裂隙、关节游离体及关节软骨丢失等典型骨关节炎改变,OARSI评分显著升高(P<0.01);其关节周围毛细淋巴管及总淋巴管增加(P<0.01和P<0.05)而成熟淋巴管减少(P<0.05);术后12周,补肾活血方组小鼠成熟淋巴管及总淋巴管均显著增加(P<0.01);其OARSI评分亦显著下降(与模型组比较,P<0.05)。结论:补肾活血方可能通过促进淋巴管成熟,增加其回流功能从而延缓关节退变。Objective:To observe the effect on regulating construction and distribution of lymphatic vessels around knee joint in osteoarthritic mice by Bushen Huoxue Formula and its therapeutic effects on degeneration of OA knees.Methods:Thirty male C57 BL/6 mice of clean grade were randomly divided into three groups(n=10 per group): the mice in sham operation group were only subjected to right knee capsulotomy; in the other two groups, animal models of osteoarthritis were established by transecting the right medial collateral ligament to open the joints space, detaching the medial meniscus from its anterior attachment to the tibia, and then removing a portion of the detached meniscus. Followed by treated with 0.2 mL/mice concentrated decoction of Bushen Huoxue Formula(Bushen Huoxue Formula group) or with the same dose of normal saline(sham and model groups) via gavage, once daily beginning at 2 days after modeling, for 12 weeks. Four and twelve weeks after treatment, respectively, half mice were sacrificed and the right knee was removed. The morphology and structure of the joint tissue were observed by Safranin-O staining; the distribution and constitution of lymphatic vessels were detected by whole-slide digital imaging following the double immunofluorescence staining.Results:In the OA model group, the articular cartilage was worn or even cracked to be loose bodies and cartilage loss, with the significant higher OARSI scores(P<0.01), total lymphatic vessels and lymphatic capillaries(P<0.01; P<0.05), and lesser mature lymphatic vessels(P<0.05). In the Bushen Huoxue Formula group, the total lymphatic vessels especially the mature lymphatics were significantly increased(P<0.01), with the decreased OARSI scores.Conclusion:Bushen Huoxue Formula via gavage can partially alleviate joint degeneration in osteoarthritis mice possibly through promoting the lymphatic maturation and improving the lymphatic function.
