慢性阻塞性肺疾病合并心力衰竭患者血清白蛋白变化及其临床意义

摘　　要：目的探讨慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)伴心力衰竭患者血清白蛋白浓度变化及其临床意义。方法选取2011年-2014年我院收治的390例慢性阻塞性肺疾病患者为研究对象,根据有无心力衰竭,分为单纯慢性阻塞性肺疾病和慢性阻塞性肺疾病合并心力衰竭二组,对二组患者血清白蛋白浓度进行比较分析,应用Logistc回归分析慢性阻塞性肺疾病合并心力衰竭的危险因素,应用Spearman相关分析血清白蛋白浓度与心力衰竭标志物血浆氨基末端B型尿钠肽前体(NT-proBNP)之间的相关性。结果慢性阻塞性肺疾病合并心力衰竭患者血清白蛋白浓度较单纯慢性阻塞性肺疾病患者明显降低(P<0.01)。Logistic回归分析结果显示血清白蛋白浓度是慢性阻塞性肺疾病合并心力衰竭的危险因素,并且血清白蛋白浓度与NT-proBNP呈明显负相关(P<0.01)。结论慢性阻塞性肺疾病患者血清白蛋白浓度降低提示并发心力衰竭,对判断慢性阻塞性肺疾病患者预后具有一定价值。Objective To investigate the changes of serum albumin and its clinical significance in patients with chronic obstructive pulmonary disease (COPD).Methods A total of 390'patients with COPD in author's hospital from 2011to 2014were chosen as the research object.All the COPD patients were divided into two groups according to with or without heart failure.Serum albumin level in these two groups was analyzed sta- tistically.Risk factors of COPD patients complicating with heart failure were analyzed by logistic regression analysis and correlation between serum albumin with NT-proBNP,a marker of heart.failure,was determined by Spearman analysis.Results The level of serum albumin in COPD patients with heart failure was significantly decreased compared with COPD patients without heart failure (P<0.01}.The results of logistic regression analysis showed that serum albumin level was a risk factor of COPD patients complicating with heart failure, and the level of serum albumin was negatively related with NT-proBNP (P<0,0l).Conclusion Significant decrease of level of serum albumin suggests possibility of complicating heart failure,which is a valuable predictor for prognosis in patients with COPD.
