综合心理干预对宫颈癌手术患者心理弹性、应对方式及生活质量的影响

摘　　要：目的探讨综合性心理干预对宫颈癌手术患者心理弹性、应对方式及生活质量的影响。方法回顾性分析2014年1月-2016年12月我院收治的107例宫颈癌手术患者的临床资料,按照资料中干预方法不同分为对照组(44例)和观察组(63例),对照组患者在所患疾病临床治疗上予以常规干预,观察组在对照组基础上予以综合心理干预,二组患者均干预4周,干预结束后比较二组患者心理弹性水平、应对方式及生活质量情况。结果①二组患者干预后心理弹性水平较干预前明显改善,观察组患者改善更明显(t=5.031,P<0.001);②观察组干预后面对因子得分明显高于对照组(t=4.976,P<0.001),回避因子、屈服因子得分明显低于对照组患者,比较差异有统计学意义(t=6.155,P<O.001;t=8.036,P<0.001);③观察组患者干预后其生活质量较对照组改善更明显,二组比较差异有统计学意义(t=6.642,P<0.001;t=4.057,P=0.004;t=4.570,P=0.001;t=4.671,P=0.010;t=6.975,P<0.001;t=4.643,P=0.002)。结论对宫颈癌手术患者实施综合心理干预的效果显著,可帮助患者积极接受现实,增强其心理弹性水平,从而达到提高生活质量的目的,可作为宫颈癌手术患者首选的干预模式。Objective To investigate the effect of comprehensive psychological intervention on mental resilience,coping style and quality of life in patients with cervical cancer.Methods The clinical data.of 107cervical cancer patients treated in author's hospital from January 2014to December 2016were retrospectively analyzed.According to the different methods .of intervention in the data,the patients were divided into the control group (n =44)and the study group (n =63).The patients in the control group received routine intervention based on clinical treatment,and the patients in the study group was given a comprehensive psychological intervention on the basis of the control group,both tid for 4weeks.After the intervention,the mental resilience, coping style and quality of life of the two groups were compared.Results The level of resilience in the two groups was significantly improved after intervention (t =2.405,P =0.018;t =8.014,P <0.001),and the improvement in the study group was more obvious (t =5.031,P<0.001).In the study group,the score of facing factor was significantly higher than that of the control group (t =4.976,P<0.001),and the score of avoidance factor and yield factor were significantly lower than those of the control group (t =6.155,P<0.001;t = 8.036,P %0.001).The improvement of the quality of life in the study group after intervention was more obvious than that of the control group,there were significant differences between the two groups (t =6.642,P<0.001;t=4.057,P=0.004;t=4.570,P=0.001;t=4.671,P=0.010;t=6.975,P<0.001;t =4.643,P = 0.002}.Conclusion Comprehensive psychological intervention is an effective way in helping the patient to accept the reality actively,enhancing their mental resilience,and achieving the purpose of improving the quality of life for patients with cervical cancer,which can be recommended as the first choice of the disease intervention.
