锁骨钩钢板与锁骨远端微孔锁定钢板在治疗锁骨远端NeerⅡ型骨折中的疗效比较

摘　　要：目的比较锁骨钩钢板与锁骨远端微孔锁定钢板在治疗锁骨远端NeerⅡ型骨折中的疗效。方法回顾性分析2015年3月—2017年5月本院收治的45例锁骨远端NeerⅡ型骨折患者,其中采用锁骨钩钢板治疗组(A组) 25例,锁骨远端微孔锁定钢板治疗组(B组) 20例。术后定期随访,按照ConstantMurley评分系统及美国肩肘外科医师学会ASAES标准评定肩关节功能。结果两组患者围手术期指标差异无显著性意义(P>0.05),但术后第1、3、6月及末次随访的Constant-Murley评分及ASAES评分中,锁骨远端微孔锁定钢板组明显更具优势(P<0.05)。结论两组内固定方式在治疗锁骨远端NeerⅡ型骨折中都取得了满意的疗效,锁骨远端微孔锁定钢板相比较锁骨钩钢板在治疗锁骨远端NeerⅡ型骨折中具有术后疼痛轻,肩关节功能好的优点,更具优势。Objective To compare the efficacy of clavicular hook plate and distal clavicle micro-locking plate in the treatment of Neer Ⅱ type fractures of the distal clavicle.Methods Retrospectively analyzed the data of 45patients with distal clavicle Neer Ⅱ type fractures treated in our department from March 2015 to May 2017, including 25 cases of clavicular hook plate in treatment group (group A),20cases in distal clavicle microlocking plate treatment (group B).After regular follow-up,the function of the shoulder joint was assessed according to the Constant-Murley scoring system and the American Academy of Shoulder and Elbow Surgeons Association (ASAES).Results There was no significant difference in perioperative indexes between the two groups(P>0.05).However,in the Constant-Murley and ASAES scores at postoperative 1,3,6 months and final follow-up,distal clavicle micro-locking plate group showed significantly advantage (P<0.05).Conclusions Both groups of internal fixations methods have achieved satisfactory results in the treatment of Neer Ⅱ type fractures of the distal clavicle.Compared with the clavicular hook plate,the distal clavicle micro-locking plate has advantages of less postoperative pain and good shoulder function in the treatment of Neer Ⅱ type fractures of the distal clavicle.
