硫酸羟氯喹对类风湿关节炎合并骨质疏松患者骨代谢的影响

摘　　要：目的探讨硫酸羟氯喹(HCQ)对类风湿关节炎(RA)合并骨质疏松(OP)患者骨代谢的影响。方法以本院门诊就诊的RA合并骨质疏松患者60例为研究对象,随机分为两组,对照组(30例)给予甲氨蝶呤(MTX)+碳酸钙,治疗组(30例)给予MTX+HCQ+碳酸钙,两组均治疗12个月。分别检测两组治疗前及治疗后血清25羟基维生素D[25(OH) D]、Ⅰ型胶原羧基末端肽(CTX)、Ⅰ型胶原氨基末端肽(NTX)的差异。结果两组患者治疗后血清25(OH) D较治疗前增加,治疗后血清CTX、NTX较治疗前下降,差异有统计学意义(P<0.05)。治疗组患者治疗后血清25(OH) D明显高于对照组,而治疗后血清CTX、NTX明显低于对照组,差异有统计学意义(P<0.05)。结论 RA合并OP患者使用HCQ治疗后能增加血清25(OH) D,降低血清CTX、NTX。HCQ可改善RA合并OP患者的骨代谢,可用于治疗RA合并OP患者。Objective To investigate the effects of hydroxychloroquine sulfate(HCQ) on the bone metabolism of patients with Rheumatoid arthritis(RA) and Osteoporosis(OP).Methods The 60 cases of outpatients were chose was research objects,those patients suffered with RA and OP and got treatment in our hospital.They were randomly divided into two groups.The control group(30 cases) was given methotrexate(MTX) plus Calcium carbonate,the treatment group(30 cases) was given MTX plus HCO and Calcium carbonate.The treatment lasted for 12 months.Serum 25(OH) D,CTX and NTX before and after treatment were compared.Results Serum 25(OH) D of two groups were higher than that before treatment,CTX and NTX were lower than before.The difference had statistical significance(P<0.05).After treatment,the serum 25(OH) D of the treatment group were significantly higher than the control group,CTX and NTX were significantly lower than the control group.The differences were statistically significant(P<0.05).Conclusions HCQ could increase the serum level of 25(OH) D and reduce CTX and NTX of patients with RA and OP.It could improve the bone metabolism of patients with RA and OP.It could be used to treat patients with RA and OP.
