摘 要:目的分析超声高清血流显像在胎儿双主动脉弓畸形中的诊断价值,为胎儿双主动脉弓畸形的诊断提供理论依据。方法选取2016年12月-2017年11月在成都市妇女儿童中心医院进行产前胎儿畸型筛查的182例孕妇为研究对象,最终确诊为双主动脉弓畸形的19例为观察组,无胎儿畸形的163例为对照组。所有孕妇均接受彩色多普勒血流显像及超声高清血流显像进行产前检查,分析彩色多普勒血流显像及超声高清血流显像的产前筛查诊断结果、灵敏度及特异度,并与两者联合检查结果进行对比分析。结果超声高清血流显像在胎儿双主动脉弓畸形筛查诊断中的灵敏度及特异度均低于联合检查,但差异无统计学意义(P>0.05);彩色多普勒血流显像在双主动脉弓畸形筛查诊断中的灵敏度及特异度均低于联合检查,差异有统计学意义(P<0.05)。结论超声高清血流显像在胎儿双主动脉弓畸形早期诊断中具有较高的应用价值,灵敏度及特异度较高,可作为双主动脉弓畸形产前筛查诊断的重要依据。Objective To analyze the value of ultrasonographic high-definition flow imaging in diagnosis of fetal double aortic arch deformity,provide a scientific basis for diagnosis of fetal double aortic arch deformity.Methods A total of 182 pregnant women receiving prenatal screening of fetal abnormalities in Chengdu Women's and Children's Central Hospital from December 2016 to November 2017 were enrolled in this study,then they were divided into observation group ( 19 cases diagnosed as fetal double aortic arch deformity definitely) and control group ( 163 cases without fetal double aortic arch deformity) .Color Doppler flow imaging and ultrasonographic high-definition flow imaging were performed among all the pregnant women during prenatal examination,the diagnostic results,sensitivities,and specificities were analyzed,then the results were compared with those of joint detection.Results The sensitivity and specificity of ultrasonographic highdefinition flow imaging in diagnosis of fetal double aortic arch deformity were lower than those of joint detection,but there was no statistically significant difference ( P>0.05) .The sensitivity and specificity of color Doppler flow imaging in diagnosis of fetal double aortic arch deformity were statistically significantly lower than those of joint detection ( P<0.05) .Conclusion The application value of ultrasonographic high -definition flow imaging in early diagnosis of fetal dual aortic arch deformity is high,the sensitivity and specificity are relatively high,which can be used as important basis for prenatal screening and diagnosis of fetal dual aortic arch deformity.
