妊娠高血压患者血管内皮素、抵抗素、脑钠肽水平与患者病情及妊娠结局的关系

摘　　要：目的探讨妊娠高血压患者血清中内皮素-1(Endothelin-1,ET-1)、抵抗素(Resistin)、脑钠肽(brain natriuretic peptide,BNP)水平与患者病情及妊娠结局的关系。方法选取本院2015年11月-2016年11月诊断为妊娠高血压的78例患者作为研究对象,所有患者均检测血清中ET-1、Resistin、BNP水平,探讨其与妊娠高血压患者病情及妊娠结局的关系,并采用Logistic回归分析探讨ET-1、Resistin、BNP水平与患者妊娠结局的关系。结果重度妊娠组患者血清中ET-1、Resistin、BNP水平均明显高于中度妊娠组和轻度妊娠组,妊娠期糖尿病妊娠结局不良组ET-1、Resistin、BNP水平均明显高于妊娠结局良好组的患者,差异均有统计学意义(P <0. 05)。Logistic回归分析结果显示,BMI指数、ET-1、重度妊娠高血压、Resistin、BNP是妊娠高血压患者不良结局的独立危险因素(P <0. 05)。结论妊娠高血压患者血管内皮素、抵抗素、脑钠肽水平是妊娠结局预后不良的独立危险因素。Objective To investigate the relationship between serum endothelin-1( ET-1),resistin,brain natriuretic peptide( BNP) and the pregnancy outcome in patients with pregnancy induced hypertension. Methods Seventy-eight patients with pregnancy induced hypertension were selected from our hospital from September 2013 to September 2016,who were conducted for the detection of serum ET-1,Resistin and BNP levels,and their relationships with the hypertension disease and pregnancy outcome were explored. Logistic regression analysis was adopted to explore the relationship between ET-1,Resistin and BNP levels and pregnancy outcome. Results Serum ET-1,Resistin and BNP levels in patients with severe pregnancy were significantly higher than that of moderate pregnancy group and mild pregnancy group; the serum ET-1,Resistin,BNP levels in gestational diabetes mellitus group with adverse pregnancy outcomes were significantly higher than that of pregnancy group with good outcome,with the differences statistically significant( P < 0. 05). Logistic regression analysis showed that BMI,ET-1,severe pregnancy hypertension,Resistin and BNP were independent risk factors for adverse outcomes in patients with pregnancy-induced hypertension( P < 0. 05). Conclusion Vascular endothelial growth factor,Resistin and brain natriuretic peptide were independent risk factors in patients with pregnancy induced hypertension.
