妊娠期女性外周血红细胞参数参考区间的调查

摘　　要：目的调查孕妇在不同孕期外周血红细胞参数的变化,建立参考区间。方法选择2016年1月-9月在本院定期接受产前检查的200例健康妊娠女性,分别记录其孕早期(13周前)、孕中期(13周~28周)、孕晚期(28周后)外周血红细胞参数的检测结果,同时收集非妊娠育龄女性186例为对照组。并另取早、中、晚各40例妊娠期女性外周血红细胞检测结果,用不同诊断标准对妊娠期贫血发生情况进行验证。结果正常妊娠组孕早、中、晚各期参考范围Hb为105 g/L^144 g/L、99 g/L^132 g/L、98 g/L^140 g/L,RBC为(3. 5~4. 8)×10^12/L、(3. 2~4. 4)×10^12/L、(3. 2~4. 8)×10^12/L,HCT为0. 31 L/L^0. 41 L/L、0. 30 L/L^0. 39 L/L、0. 28 L/L^0. 41 L/L,RDW-CV为12. 0%~14. 8%、11. 2%~15. 3%、11. 9%~15. 6%。妊娠组的Hb、RBC、HCT均低于对照组(P <0. 05),其中孕中期最低。结论本地孕妇妊娠期贫血的诊断参考值低于WHO标准及美国CDC标准,建立妊娠女性参考区间可更好地评估孕妇的健康水平。Objective To investigate the changes of blood erythrocyte parameters in different pregnancy,and to establish a normal reference interval. Methods A total of 200 healthy pregnant women receiving regular antenatal examination were selected during January-September 2016 from our hospital,and the detection results of peripheral blood erythrocyte parameters in the early pregnancy( 13 weeks ago),mid pregnancy( 13 to 28 weeks),late pregnancy( 28 weeks later) were recorded. At the same time,186 cases at no child-bearing age were collected as control group. The incidence of pregnancy anemia was confirmed by different diagnostic criteria in the early,middle and late pregnancies( 40 cases in each group). Results The reference ranges of normal gestation pregnancy,middle and late stage were as follows: Hb by 105 g/L-144 g/L,99 g/L-132 g/L,98 g/L-140 g/L,RBC by( 3. 5-4. 8) × 1012/L,( 3. 2-4. 4) × 1012/L,( 3. 2-4. 8) × 1012/L,HCT by 0. 31 L/L-0. 41 L/L,0. 30 L/L-0. 39 L/L,0. 28 L/L-0. 41 L/L,RDW-CV by 12. 0%-14. 8%,11. 2%-15. 3%,11. 9%-15. 6%. Hb,RBC and HCT in pregnancy group were all lower than that in the control group,and the differences were statistically significant( P < 0. 05). Hb,RBC and HCT were the lowest in the middle of pregnancy. Conclusion The diagnostic reference value of anemia in local pregnant women is lower than the WHO standard and the U. S. CDC standard,so establishing a reference interval for pregnant women can better assess the health of pregnant women.
