摘 要:目的 :探讨不同原因所致缺血性坏死股骨头的病理改变及碱性成纤维细胞生长因子(bFGF)的表达。方法 :选取2015年1月至2018年12月期间四川省骨科医院收治的40例股骨头缺血性坏死患者作为研究对象。这些患者均使用人工全髋关节置换术进行治疗。将10例股骨颈骨折所致股骨头缺血性坏死患者作为对照组。将30例非创伤性股骨头缺血性坏死患者作为实验组。将实验组中10例激素性股骨头缺血性坏死患者作为激素组,将10例酒精性股骨头缺血性坏死患者作为酒精组,将10例特发性股骨头缺血性坏死患者作为特发组。观察这些患者股骨头的病理改变及bFGF的表达情况。结果 :激素组患者、酒精组患者及特发组患者股骨头坏死区bFGF阳性面积的百分比低于其增生区及对照组患者的对应区域bFGF阳性面积的百分比,其增生区bFGF阳性面积的百分比高于对照组患者的对应区域bFGF阳性面积的百分比,P <0.05。结论 :不同原因所致股骨头缺血性坏死患者股骨头的病理改变及bFGF的表达均不相同。临床医生可根据股骨头缺血性坏死患者的病理改变及bFGF的表达情况对其进行合理的细胞因子治疗。objective: to investigate the pathological changes of femoral head caused by ischemic necrosis and the expression of basic fibroblast growth factor(bFGF). Methods: 40 cases of avascular necrosis of femoral head admitted by sichuan orthopaedic hospital from January 2015 to December 2018 were selected as subjects. All of these patients were treated with total hip arthroplasty. Ten patients with ischemic necrosis of femoral head caused by femoral neck fracture were treated as the control group. 30 cases of non-traumatic avascular necrosis of femoral head were treated as experimental group. In the experimental group, 10 cases of steroid-induced ischemic necrosis of femoral head were treated as the hormone group, 10 cases of alcoholic avascular necrosis of femoral head were treated as the alcohol group, and 10 cases of idiopathic avascular necrosis of femoral head were treated as the idiopathic group. The pathological changes of femoral head and the expression of bFGF in these patients were observed. Results: patients with hormone group and alcohol group of patients, and femoral head necrosis in patients with idiopathic group bFGF positive area percentage points below its proliferation areas and the control group in patients with corresponding bFGF positive area percentage, its proliferation zone bFGF positive area percentage of corresponding area is higher than the control group patients bFGF positive area percentage, P < 0.05). Conclusion: the pathological changes of femoral head and the expression of bFGF were different in patients with ischemic necrosis of femoral head caused by different reasons. The clinician can carry out reasonable cytokine treatment according to the pathological changes and the expression of bFGF in patients with ischemic necrosis of femoral head.
